【摘要】：目的:通過觀察參蛭通心膠囊合葶藶大棗瀉肺湯對氣虛血瘀,陽虛水泛型慢性收縮性心力衰竭的臨床治療效果。方法:遵循隨機(jī)、對照的原則,選取山東中醫(yī)藥大學(xué)附屬醫(yī)院心內(nèi)科慢性收縮性心力衰竭患者60例,隨機(jī)分成試驗(yàn)組30例和對照組30例,根據(jù)2014年版《中國心力衰竭診斷和治療指南》,常規(guī)給以ACEI(血管緊張素轉(zhuǎn)換酶抑制劑)治療,對ACEI不耐受者給與ARB(血管緊張素受體拮抗劑)治療,以及β-受體阻滯劑(β-receptor blocker)、醛固酮受體拮抗劑、利尿劑、強(qiáng)心甙類藥物等西藥治療,試驗(yàn)組除給與西藥治療外,加用參蛭通心膠囊合葶藶大棗瀉肺湯治療,2組均治療4周觀察療效。觀察治療前后的中醫(yī)證侯積分、LVEF(左心室射血分?jǐn)?shù))、NT-proBNP(N末端腦鈉肽原)、6MWT(6分鐘步行距離)、心功能分級等變化。結(jié)果:兩組的中醫(yī)證侯積分、LVEF、NT-proBNP、6MWT、心功能分級均有改善,試驗(yàn)組的療效明顯優(yōu)于對照組,兩者具有統(tǒng)計(jì)學(xué)差異(P0.05).60例患者在治療前后血常規(guī)和肝腎功檢測均無異常。結(jié)論:參蛭通心膠囊合葶藶大棗瀉肺湯可改善氣虛血瘀,陽虛水泛型慢性心力衰竭患者臨床癥狀,提高患者的生活質(zhì)量,安全性和依從性好。
[Abstract]:Objective: to observe the clinical effect of Shenzhi Tongxin capsule combined with Tinglidazaoxiefeng decoction on chronic systolic heart failure with Qi deficiency and blood stasis, Yang deficiency water generality. Methods: to follow the principle of randomness and control. Sixty patients with chronic systolic heart failure (CHF) in Department of Cardiology, affiliated Hospital of Shandong University of traditional Chinese Medicine, were randomly divided into experimental group (n = 30) and control group (n = 30). According to the 2014 edition of the guidelines for the diagnosis and treatment of Heart failure in China, ACEI (angiotensin converting enzyme inhibitor) is routinely administered. Patients with ACEI intolerance were treated with ACEI (angiotensin receptor antagonist) and 尾 -receptor blocker (尾 -receptor blocker). Diuretics, cardioside drugs and other western medicine treatment, the experimental group in addition to western medicine treatment, plus Shenzhi Tongxin capsule and Tingdazao Xiefei decoction treatment. The therapeutic effect was observed for 4 weeks in both groups, and the LVEF (left ventricular ejection fraction) was observed before and after treatment, and the NT-proBNPN N-terminal brain natriuretic peptide (NT-proBNPN) was observed. Results: LVEFN NT-proBNPN 6MWT6 MWT6 minutes walking distance, cardiac function grading and so on. Results: the score of LVEFNT-proBNPN 6MWTwas improved in both groups. The curative effect of the test group was better than that of the control group. There was statistical difference between the two groups. 60 patients had no abnormal blood routine and liver and kidney function before and after treatment. Conclusion: Shenzhi Tongxin capsule combined with Tingli Da Zao Xiefei decoction can improve qi deficiency and blood stasis. The clinical symptoms of patients with chronic heart failure due to yang deficiency and water deficiency can improve their quality of life, safety and compliance.
