【摘要】：目的：1.文獻研究：通過搜集近30年有關(guān)膈下逐瘀湯在婦科臨床應用方面的文獻,對其進行統(tǒng)計分析,進一步歸納總結(jié)出膈下逐瘀湯在婦科臨床方面的應用規(guī)律,為臨床工作提供參考,提高臨床的療效。2.臨床觀察：觀察膈下逐瘀湯加減治療慢性盆腔炎氣滯血瘀證的臨床療效。方法：1.收集1985年1月至2015年10月在清華同方中國科技期刊數(shù)據(jù)庫、萬方數(shù)據(jù)庫、維普數(shù)據(jù)庫上發(fā)表的有關(guān)膈下逐瘀湯在婦科臨床應用方面的文獻。利用現(xiàn)代化統(tǒng)計工具對搜索到的文獻加以處理,建立相關(guān)數(shù)據(jù)庫,從多個角度進行處理分析,總結(jié)其運用范圍、病因病機、適用證型、功效、常用的加減化裁以及配合的其它療法等信息,將文獻數(shù)據(jù)轉(zhuǎn)換為數(shù)字信息,并對結(jié)果進行歸納分析。2.收集氣滯血瘀型慢性盆腔炎患者50例,給予膈下逐瘀湯加減口服治療,觀察其治療前后癥狀、局部體征的變化情況,超聲監(jiān)測盆腔包塊的變化。結(jié)果：1.運用膈下逐瘀湯治療婦科疾病的現(xiàn)代文獻中,痛經(jīng)出現(xiàn)的頻率最高,占41.49%,其次為盆腔炎11.70%；常見的癥狀為腹痛,占87.09%；其涉及的病因病機以氣滯血瘀、氣血不暢最多見,分別占56.18%、22.47%；其加減涉及的功效以活血化瘀為最多占59.34%,行氣止痛次之,為32.97%；在藥味加減使用方面,膈下逐瘀湯加味的藥物共有63種,分屬15類,其中以活血化瘀藥及補虛藥最多,其次為清熱藥；與其合用以治婦科疾病的成方有當歸芍藥散和大柴胡湯；配合的外治療法多樣,以中藥外敷和針刺為主。2.研究過程中剔除5例,脫落2例,剩余43例。治療后綜合療效總有效率為81.40%,中醫(yī)癥狀療效總有效率為86.05%,局部體征療效總有效率為72.09%；治療前后中醫(yī)癥狀積分比較差異顯著(P0.01),治療前后局部體征積分比較差異顯著(P0.01),治療后盆腔包塊消散明顯。結(jié)論：1.(1)現(xiàn)代婦科運用膈下逐瘀湯主要以疼痛性疾病和積聚為主,脈象多為弦脈、弦澀脈,多見舌紫黯、有瘀斑瘀點,苔薄白；(2)本方所主病因病機主要為氣滯血瘀；功效主要為活血化瘀、行氣止痛；(3)本方加味的藥物主要為活血化瘀、補虛、清熱、溫里等類；主要配合的外治療法為中藥外敷和針刺。2.膈下逐瘀湯加減能夠顯著改善氣滯血瘀型慢性盆腔炎患者的臨床癥狀和局部體征,并能促進盆腔包塊的吸收,是治療氣滯血瘀型慢性盆腔炎的有效方藥。

Conclusion: 1. (1) by using modern gynecological gexiazhuyu Decoction mainly in pain disease and accumulation, pulse string pulse, unsmooth pulse string, see dark purple tongue, ecchymosis petechiae, moss thin white; (2) the main pathogenesis is Qi stagnation and blood stasis; the main effect of promoting blood circulation and removing blood stasis, Qi pain; (3) the main prescription drugs For blood stasis, tonifying deficiency, heat, temperature etc.; mainly with external therapy for external application of Chinese medicine and acupuncture.2.
