【摘要】:目的:舌診是中醫(yī)望聞問切四診合參的重要組成部分,是辨證論治的基礎(chǔ),對中醫(yī)診斷疾病、辨證分型、遣方用藥、判定預(yù)后具有重要的指導(dǎo)意義。近年來,對于舌診客觀化以及與疾病相關(guān)性分析的研究越來越多。本研究以初發(fā)2型糖尿病厚苔患者為研究對象,探討糖尿病厚苔形成相關(guān)的調(diào)節(jié)蛋白分子機(jī)制。方法:于中國中醫(yī)科學(xué)院廣安門醫(yī)院招募初發(fā)2型糖尿病厚苔以及薄苔患者共72名,以及健康人群厚苔以及薄苔者60名為對照組,留取空腹血清行ELISA檢測EGF、E-cad等蛋白含量,留取舌苔標(biāo)本行PCR檢測EGF、E-cad等基因含量,分析組間相關(guān)性以及與血糖、血脂、BMI水平的相關(guān)性。另招募初發(fā)糖尿病患者6例,其中厚苔3例薄苔3例,以及6名健康人厚薄苔對照組,留取舌苔標(biāo)本行16s rDNA微生物測序,檢測糖尿病患者與健康人不同厚薄舌苔樣本中微生物菌群豐度以及多樣性。結(jié)果:1、血清ELISA檢測:2型糖尿病厚苔和薄苔患者中,E-cad表達(dá)水平具有統(tǒng)計學(xué)意義;健康人薄厚苔中,E-cad與EGF表達(dá)水平具有統(tǒng)計學(xué)意義;2型糖尿病厚苔與健康人厚苔,E-cad與EGF表達(dá)水平具有統(tǒng)計學(xué)意義;2型糖尿病薄苔與健康人薄苔,E-cad表達(dá)水平具有統(tǒng)計學(xué)意義;糖尿病厚苔組與健康厚苔組間CH0、TG、HDL、LDL比較差異有統(tǒng)計學(xué)意義;糖尿病薄苔與健康薄苔組間CHO、TG、LDL比較差異有統(tǒng)計學(xué)意義;糖尿病厚苔與健康組薄苔之間CH0、TG、LDL比較差異有統(tǒng)計學(xué)意義;BMI值比較,糖尿病厚、薄苔組間、糖尿病厚苔與健康對照厚苔組間、糖尿病厚苔組與健康薄苔組間比較,差異均有統(tǒng)計學(xué)意義。2、PCR檢測:E-cad在糖尿病厚苔組與薄苔組之間具有統(tǒng)計學(xué)意義,在糖尿病薄苔中E-cad含量含量低,厚苔中含量高;同時發(fā)現(xiàn)目標(biāo)基因包括內(nèi)參GAPDH于舌苔中含量較低。3、16s rDNA測序共檢測出130余個OTUs,初步統(tǒng)計過后,發(fā)現(xiàn)Granulicatella在糖尿病薄苔比健康人厚苔和薄苔含量高、健康人薄苔比健康人厚苔含量高;Rothia在健康人厚苔中和糖尿病厚苔患者中明顯偏高。結(jié)論:E-cad在2型糖尿病、薄苔患者中高表達(dá),說明E-cad可能與糖尿病以及厚苔形成具有相關(guān)性;綜合ELISA血清檢測以及PCR舌苔檢測發(fā)現(xiàn),E-cad對于糖尿病以及舌苔厚度具有一定的差異性;血脂、BMI與糖尿病相關(guān),與舌苔厚薄無關(guān);Granulicatella、Rothia在舌苔微生物菌群中具有差異性,具體機(jī)制有待進(jìn)一步研究。
[Abstract]:Objective: tongue diagnosis is an important part of traditional Chinese medicine, which is the basis of syndrome differentiation and treatment. It has an important guiding significance in the diagnosis of diseases, syndrome differentiation, prescription medication and prognosis in recent years. There are more and more researches on the objectification of tongue diagnosis and the analysis of correlation between tongue diagnosis and disease. This study focuses on the patients with thick moss of primary type 2 diabetes mellitus (T2DM). Methods: 72 patients with type 2 diabetes mellitus were recruited in Guanganmen Hospital of Chinese Academy of traditional Chinese Medicine. And 60 healthy people with thick and thin fur were taken as control group. Fasting serum was taken to detect the content of EGF-E-cad and PCR was used to detect EGF in tongue coating samples. The content of E-cad gene, the correlation between the analysis group and the blood sugar, blood lipid and BMI were analyzed. 6 patients with primary diabetes mellitus were recruited, including 3 cases of thick fur and 3 cases of thin fur. The specimens of tongue coating were collected from 6 healthy subjects and the control group were collected. The samples were sequenced by 16s rDNA microorganism. The microbial flora abundance and diversity in different thick and thin tongue coating samples from diabetic patients and healthy people were detected. Results: 1. Serum ELISA was used to detect thick fur and thin fur patients with type 2 diabetes mellitus. The expression level of E-cad was statistically significant. The expression levels of E-cad and EGF in healthy persons were statistically significant. The expression of E-cad and EGF in type 2 diabetes mellitus was significantly higher than that in healthy subjects. The expression of E-cad in type 2 diabetes mellitus was significantly higher than that in healthy subjects. There was a significant difference in HDLL-LDL between the CH0 TGG and the healthy thick fur group in the diabetes mellitus (DM) group and the control group (P < 0.05). There was significant difference in TGG LDL between diabetic thin fur group and healthy thin fur group. There was a significant difference in TGG LDL between the thick fur of diabetes mellitus and the thin fur of the healthy group. There were significant differences in BMI values between the diabetic thick fur group and the thin fur group, between the diabetic thick fur group and the healthy control group, and between the diabetic thick fur group and the healthy thin fur group. PCR analysis showed that the content of E-cad was lower in the diabetic thin fur and higher in the thick fur than that in the thin one. At the same time, it was found that the target gene, including the content of GAPDH in tongue coating, was low. 3C16 s rDNA sequencing detected more than 130 OTUs. After preliminary statistics, more than 130 OTUs were detected. It was found that the content of Granulicatella in diabetic thin fur was higher than that in healthy persons, and that in healthy persons was higher than that in healthy persons. Rothia was significantly higher in healthy persons with thick fur and diabetic patients with thick fur. Conclusion the expression of E-cad is high in type 2 diabetes mellitus and thin fur patients. These results suggest that E-cad may be associated with diabetes mellitus and the formation of thick moss. Combined with ELISA serum test and PCR tongue coating test, it was found that E-cad had some difference in diabetes mellitus and tongue coating thickness. BMI of blood lipids was correlated with diabetes mellitus, but not with thickness of tongue coating. Granulicatellaa Rothia is different in the microflora of tongue coating, and the specific mechanism needs further study.
