【摘要】：評價《腫瘤中醫(yī)診療指南》的適用性和臨床應用情況。評價方法包括適用性評價和應用性評價。適用性評價采用調(diào)查問卷方法,評價《指南》熟悉和使用情況、質(zhì)量水平、臨床適用情況等;應用性評價采用前瞻性病例調(diào)查分析方法,評價《指南》臨床應用符合度和應用效果(包括療效、安全性、經(jīng)濟性)。采用統(tǒng)計學基本描述、構建不同比較組進行交叉或分層統(tǒng)計學檢驗、多因素分析以及混雜因素處理等多種分析方法。適用性綜合性評價結果顯示,《指南》內(nèi)容完全適用和基本適用者為63.03%,不適用者4.24%;應用性綜合評價結果顯示,《指南》"總體療效與技術水平"、"滿意度"、"在臨床實踐中的適用性"較好的累計統(tǒng)計分別為85.46%,80.43%,69.40%。該研究表明基地單位醫(yī)師對《腫瘤中醫(yī)診療指南》的認知度較高,低年資醫(yī)師是主要使用群體,臨床指南總體臨床適用、臨床應用符合度較好,但《指南》的診療方案的質(zhì)量和適應性等尚需改進。指南在支持證據(jù)、方法學規(guī)范及臨床使用性上有所欠缺,應盡快通過修訂進一步提高質(zhì)量。
[Abstract]:To evaluate the applicability and clinical application of the guidelines for the diagnosis and treatment of Cancer in TCM. The evaluation methods include applicability evaluation and application evaluation. The applicability evaluation adopts questionnaire method to evaluate the familiarity and use of the guidelines. Quality level, clinical application and so on; The prospective case investigation and analysis method was used to evaluate the degree of conformity and effect of clinical application (including curative effect, safety and economy). Different comparison groups were constructed to carry out cross or stratified statistical tests, multivariate analysis and hybrid factor processing. The applicability of comprehensive evaluation results showed. The content of the Guide is fully applicable and basic applicable to 63.03, not applicable to 4.24; The results of application comprehensive evaluation showed that the cumulative statistics of "overall curative effect and technical level", "satisfaction" and "applicability in clinical practice" were 85.46% and 80.43% respectively. 69.40. the research shows that the doctors of base units have a high degree of recognition of "Cancer TCM Diagnostic and treatment guidelines", the junior doctors are the main use group, the clinical guidelines are generally applicable, and the clinical application coincidence is good. However, the quality and adaptability of the guidelines need to be improved. The guidelines are deficient in supporting evidence, methodological specifications and clinical usability, and should be further improved through revision as soon as possible.
