中醫(yī)傳統(tǒng)運動處方治療代謝綜合征的臨床觀察

【摘要】:背景:隨著現(xiàn)代社會人口老齡化,生活方式的改變,代謝綜合征的發(fā)病率逐年增加,已經(jīng)成為全球公共衛(wèi)生議題,嚴重威脅人類健康。不良生活方式對代謝綜合征的發(fā)生及發(fā)展有密切關(guān)系,久坐少動是文明社會中生活方式的特點,諸多研究強調(diào)運動對代謝綜合征的積極作用,探索有效的運動方式,于代謝綜合征的防治是一項必要的研究。目的:本研究以代謝綜合征患者為研究對象,應(yīng)用一套中醫(yī)傳統(tǒng)運動處方的進行治療,將此治療方法推廣至臨床實踐,以期控制疾病進展、疏導情志、提高患者生活質(zhì)量,提高臨床上治療代謝綜合征的療效。方法:根據(jù)首都國醫(yī)名師呂仁和教授及臺灣省太極拳協(xié)會理事長陳漢玉老師的傳統(tǒng)運動養(yǎng)生經(jīng)驗,選擇,制定一套中醫(yī)傳統(tǒng)運動處方。將該運動處方用于干預(yù)代謝綜合征患者,進行12周臨床運動試驗,用前后對照的方式,觀察其在控制體重、調(diào)節(jié)血糖、改善胰島素抵抗、調(diào)節(jié)血脂及改善中醫(yī)癥狀等各方面的作用。結(jié)果:經(jīng)過12周中醫(yī)傳統(tǒng)運動鍛煉后,BMI下降0.19±0.47kg/m2,腰圍減少1.26±1.83cm,腰臀比下降0.02±0.17,并且腰圍和腰臀比男性改善較女性明顯,患者未有明顯的飲食干預(yù),呈現(xiàn)較平穩(wěn)安全的下降趨勢。空腹血糖下降0.83±0.94mmol/L,達標率從42%提升到85%,糖化血紅蛋白下降0.33±0.56%,達標率由42%提升到57%,說明中醫(yī)傳統(tǒng)運動對于血糖的控制有較理想的效果。干預(yù)前后31例患者空腹胰島素雖無明顯變化,但對胰島素分泌水平較高的患者有明顯療效,治療前后空腹胰島素下降13.69±14.18mU/L,說明中醫(yī)傳統(tǒng)運動可改善胰島素抵抗現(xiàn)象,緩解高胰島素血癥。血脂方面,治療后甘油三酯總體下降0.20±0.61mmol/L,超標患者下降0.64±0.58mmol/L,由此可見中醫(yī)傳統(tǒng)運動對降低甘油三酯有明顯療效。12周的干預(yù)對于總體低密度脂蛋白未見明顯改善,總體下降0.13±0.49mmol/L,但對于低密度脂蛋白超標患者,則下降0.29±0.38mmol/L,前后有顯著差異,說明中醫(yī)傳統(tǒng)運動具有降低低密度脂蛋白的作用。該試驗中總膽固醇及高密度脂蛋白前后無統(tǒng)計學意義,可能與患者未積極配合其他生活方式干預(yù)或治療時間不足有關(guān)。中醫(yī)癥狀方面,整體有明顯療效,33個癥狀中有27個得到改善。經(jīng)過12周中醫(yī)傳統(tǒng)運動治療后,中醫(yī)癥狀積分下降4.81 ±3.62,療效最明顯的癥狀為心悸、倦怠乏力、腰背痛、手足心熱、大便干燥、胸脅痛、多食易饑、口苦、肢體疼痛、小便頻多,其次為浮腫、口渴喜飲、失眠、眩暈、胸悶等。結(jié)論:經(jīng)31例患者試驗前后對照,發(fā)現(xiàn)中醫(yī)傳統(tǒng)運動對于控制身體質(zhì)量指數(shù)及腰圍、改善胰島素抵抗、降低空腹血糖及糖化血紅蛋白,調(diào)節(jié)血脂代謝,并能緩解代謝綜合征的中醫(yī)臨床癥狀。證實中醫(yī)傳統(tǒng)運動能延緩代謝綜合征疾病發(fā)展,改善相關(guān)指標及其癥狀。
[Abstract]:Background: with the aging of population and the change of life style, the incidence of metabolic syndrome is increasing year by year, which has become a global public health issue. Bad lifestyle is closely related to the occurrence and development of metabolic syndrome. Sedentary and less active is the characteristic of lifestyle in civilized society. Many studies emphasize the positive effect of exercise on metabolic syndrome, and explore the effective exercise mode, and it is necessary to study the prevention and treatment of metabolic syndrome. Objective: this study focused on the patients with metabolic syndrome. A set of traditional Chinese medicine exercise prescription was used to treat the patients, and the treatment method was extended to clinical practice, in order to control the progress of the disease, dredge feelings, improve the quality of life of patients. Methods: according to the traditional exercise experience of Professor Lu Renhe, a famous medical teacher in the capital country, and teacher Chen Hanyu, chairman of Taijiquan Association of Taiwan Province. To establish a set of traditional Chinese medicine exercise prescription. The exercise prescription was used to intervene the patients with metabolic syndrome. The 12-week clinical exercise test was carried out to observe the control of body weight and blood sugar regulation. Results: after 12 weeks of traditional Chinese medicine exercise, BMI decreased 0.19 鹵0.47 kg / m2. Waist circumference decreased 1.26 鹵1.83 cm, waist-to-hip ratio decreased 0.02 鹵0.17, waist circumference and waist-hip ratio improved significantly in males than in females, and there was no significant dietary intervention. The fasting blood glucose decreased by 0.83 鹵0.94 mmol / L, the standard rate increased from 42% to 85 and the glycosylated hemoglobin decreased by 0.33 鹵0.56%. The rate of reaching the standard was raised from 42% to 57, indicating that traditional Chinese medicine exercise had a better effect on the control of blood glucose. There was no significant change in fasting insulin in 31 patients before and after intervention. Before and after treatment, fasting insulin decreased 13.69 鹵14.18mU / L, indicating that traditional Chinese medicine exercise can improve the phenomenon of insulin resistance. To relieve hyperinsulinemia, triglyceride decreased by 0.20 鹵0.61mmol / L after treatment, and patients with hyperinsulinemia decreased by 0.64 鹵0.58mmol / L after treatment. It can be seen that traditional Chinese medicine exercise has a significant effect on triglyceride reduction. 12 weeks of intervention on the overall low density lipoprotein did not significantly improve, the overall decline of 0.13 鹵0.49 mmol / L. However, in patients with low density lipoprotein (LDL) over standard, the decrease was 0.29 鹵0.38 mmol / L, and there was significant difference between the two groups. It shows that traditional Chinese medicine exercise has the effect of reducing low density lipoprotein. There is no statistical significance before and after total cholesterol and high density lipoprotein in this experiment. Patients may not actively cooperate with other lifestyle intervention or treatment of insufficient time related. Traditional Chinese medicine symptoms, the overall effect is obvious. After 12 weeks of traditional Chinese medicine exercise treatment, the TCM symptom score decreased by 4.81 鹵3.62. The most obvious symptoms were palpitation, fatigue and low back pain. Hot hands and feet, dry stool, chest pain, more hunger, bitter mouth, limb pain, frequent urination, followed by swelling, thirst, insomnia, vertigo, chest tightness, etc. Conclusion: 31 patients before and after the trial control. It is found that traditional Chinese medicine exercise can control body mass index and waist circumference, improve insulin resistance, reduce fasting blood glucose and glycosylated hemoglobin, regulate blood lipid metabolism. It is proved that traditional Chinese medicine exercise can delay the development of metabolic syndrome disease and improve the related indexes and symptoms.
