【摘要】：目的:觀察益氣養(yǎng)陰,清熱利濕為主要作用的參麥芩苡湯對(duì)中虛濕熱型慢性萎縮性胃炎的臨床療效,并探討其組方原理與作用機(jī)制,以期進(jìn)一步總結(jié)與推廣名老中醫(yī)單兆偉教授治療慢性萎縮性胃炎的經(jīng)驗(yàn)。方法:選取2014年10月至2016年09月江蘇省中醫(yī)院名醫(yī)堂門診慢性萎縮性胃炎患者60例,所有患者中醫(yī)辨證屬于中虛濕熱證。予參麥芩苡湯治療6個(gè)月,觀察治療前后主要癥狀、胃鏡及病理變化情況。結(jié)果:經(jīng)治療后復(fù)查胃鏡及病理,60例萎縮治愈24例,治愈率40.0%,總有效率73.3%;56例腸化治愈14例,治愈率25.0%,總有效率57.1%。參麥芩苡湯加減對(duì)HP感染總有效率為26.1%,對(duì)黏膜糜爛總有效率為38.9%,對(duì)膽汁反流總有效率為63.2%。60例CAG患者治療前后中醫(yī)證候療效進(jìn)行比較,臨床治愈0例,顯效10例,有效43例,無效7例,總有效率88.3%。結(jié)論:以益氣養(yǎng)陰,清熱利濕為主要作用的參麥芩苡湯對(duì)中虛濕熱型慢性萎縮性胃炎療效明顯,能明顯改善患者胃鏡病理指標(biāo)及臨床癥狀。
[Abstract]:Objective: to observe the clinical effect of Shenmai Qin Yi decoction which is mainly used to nourish qi and nourish yin and clear heat and heat for the treatment of chronic atrophic gastritis with central deficiency and dampness heat, and to explore the principle and mechanism of its composition. To further summarize and promote the old Chinese medicine professor Shan Zhaowei's experience in the treatment of chronic atrophic gastritis. Methods:. From October 2014 to September 2016, 60 patients with chronic atrophic gastritis were selected. All patients were treated with Shenmaicalin decoction for 6 months. The main symptoms, gastroscopy and pathological changes were observed before and after treatment. Results: after treatment, gastroscopy and pathology were reviewed. Among 60 cases, 24 cases were cured by atrophy, the cure rate was 40.0%, and the total effective rate was 73.3%. 14 cases were cured with intestinal metaplasia, the cure rate was 25.0 and the total effective rate was 57.1. The total effective rate of Shenmaicenyi decoction for HP infection was 26.1cm, and the total effective rate for mucosal erosion was 38.9%. The total effective rate of bile regurgitation was 63.2%. 60 cases of CAG patients were compared before and after treatment. The clinical curative effect was 0 cases, remarkable effect 10 cases, effective 43 cases, ineffective 7 cases. Conclusion: Shenmai Qinyi decoction with the main effect of supplementing qi and nourishing yin, clearing heat and promoting dampness is effective in treating chronic atrophic gastritis with central deficiency, dampness and heat. It can obviously improve the pathological index and clinical symptoms of gastroscopy.
