培元固本治法的理論探討及其對腎虛型AD大鼠新生神經(jīng)元的作用研究

【摘要】:目的:元氣是維持生命活動的基礎(chǔ)物質(zhì),是人體生命活動的原動力。元氣稟受于父母先天之精,藏之于腎和命門,F(xiàn)代流行病學(xué)研究也表明,AD患者中以腎虛證最為常見。中醫(yī)認為“腎虛髓空”是AD的根本,并把“從腎論治”作為主要治療法則。本課題從培補元氣出發(fā),以腎虛型老年癡呆為切入點,探討元氣與腎虛型老年癡呆的關(guān)系以及培元固本治法對腎虛型老年癡呆的作用。以腎虛型老年癡呆大鼠為動物模型,觀察培元固本復(fù)方長生固本方、固本健腦液及培元固本膠囊對腎虛型癡呆模型大鼠海馬內(nèi)Aβ_(1-40)、Aβ_(1-42)的聚集情況以及齒狀回新生神經(jīng)元增值和成熟的研究,旨在為臨床治療腎虛型老年癡呆提供理論依據(jù)和實驗數(shù)據(jù)。方法:1.理論探討:收集整理元氣與腎虛型老年癡呆的相關(guān)文獻以及癡呆患者海馬內(nèi)新生神經(jīng)元增殖及成熟的相關(guān)文獻,探討從培補元氣論治腎虛型AD的理論依據(jù)。2.實驗研究:以腎虛型AD大鼠為動物模型,以海馬內(nèi)新生神經(jīng)元增值與成熟的情況為探討對象,來研究培元固本中藥復(fù)方長生固本方、固本健腦液及培元固本膠囊對腎虛型癡呆模型大鼠的作用。用體重、皮質(zhì)酮、睪酮和水迷宮的數(shù)據(jù)評價腎虛型老年癡呆模型;用Morris水迷宮定位航行實驗和空間探索實驗檢測模型大鼠的學(xué)習(xí)記憶能力;ELISA法檢測固本三方對大鼠海馬組織Aβ_(1-40)和Aβ_(1-42)蛋白含量的影響;Brdu免疫組化分析固本三方對腎虛型AD大鼠模型新生神經(jīng)元增值情況。結(jié)果:1.體重及皮質(zhì)酮、睪酮含量變化:與空白組比較,模型組大鼠體重隨著氫化可的松注射沒有增重甚至減輕;與空白組比較,模型組血清中皮質(zhì)酮含量明顯低于空白組(p0.05);與空白組比較,模型組血清中睪酮含量顯著低于空白組(p0.01)。結(jié)合體重及一般情況,腎虛模型造模成功。2.Morris水迷宮測試結(jié)果顯示:與模型組比較,各組大鼠上平臺潛伏期均縮短(p0.01,p0.05),游泳總路程均減少(p0.01,p0.05),與多奈哌齊組比較,長生固本方組、固本健腦液組上平臺潛伏期和游泳總路程都沒有明顯差異(p0.05),培元固本膠囊組上平臺潛伏期無明顯變化(p0.05),游泳總路程增加(p0.05);相比模型組,其余各組大鼠的穿越平臺總數(shù)量及目標象限時間都顯著增多(p0.01,p0.05),第一次穿越平臺時間均明顯縮短(p0.01,p0.05)。與多奈哌齊組比較,長生固本方組大鼠在固定象限停留時間及第一次抵達平臺時間均沒有明顯區(qū)別(p0.05),經(jīng)過平臺區(qū)的次數(shù)相對減少(p0.05);固本健腦液組大鼠穿越平臺次數(shù)及目標區(qū)域總時間沒有明顯區(qū)別(p0.05),第一次抵達平臺時間相對較長(p0.01);培元固本膠囊組穿越平臺時間無差異(p0.05),目標區(qū)域總時間縮短(p0.05),第一次抵達平臺時間相對較長(p0.05)。3.大鼠海馬組織Aβ_(1-40)和Aβ_(1-42)蛋白的含量:與比模型組,空白組、多奈哌齊組、長生固本方組大鼠海馬組織內(nèi)Aβ_(1-40)含量明顯降低(p0.01),固本健腦液組和培元固本膠囊組大鼠海馬組織內(nèi)Aβ_(1-40)含量降低(p0.05);與多奈哌齊組比較,長生固本方組Aβ_(1-40)含量無明顯異常(p0.01),固本健腦液組和培元固本膠囊組Aβ_(1-40)含量升高(p0.05);相比模型組,空白組、多奈哌齊組、長生固本方組、固本健腦液組大鼠海馬組織內(nèi)Aβ_(1-42)含量明顯降低(p0.01),培元固本膠囊組大鼠海馬組織內(nèi)Aβ_(1-42)含量降低(p0.05);與多奈哌齊組比較,長生固本方組、固本健腦液組Aβ_(1-42)含量無明顯異常(p0.01),培元固本膠囊組Aβ_(1-42)含量升高(p0.05)。4.新生神經(jīng)元成熟細胞數(shù)結(jié)果比較:與模型組比較,空白組、多奈哌齊組、長生固本方組和固本健腦液組其新生神經(jīng)元成熟細胞數(shù)增多(p0.05),而培元固本膠囊組新生神經(jīng)元成熟細胞數(shù)未見明顯區(qū)別(p0.05)。與多奈哌齊組比較,長生固本方組、固本健腦液組及培元固本膠囊組無差異(p0.05)。結(jié)論:1.造模結(jié)束后,模型組大鼠體重增加不明顯,甚至出現(xiàn)體重下降,活躍程度低,不愛活動,便稀軟,顫栗,被毛炸開并脫落明顯等現(xiàn)象。隨著藥物的延續(xù),逐漸出現(xiàn)活動度差、便溏、怕冷、豎毛、毛發(fā)光澤度差等癥狀;結(jié)合記憶學(xué)習(xí)能力水平大幅度下降,判斷本實驗造模成功。2.長生固本方、固本健腦液及培元固本膠囊均能在一定程度上改善模型大鼠的記憶學(xué)習(xí)功能,并且無明顯差異。3.長生固本方、固本健腦液及培元固本膠囊均能在一定程度上降低海馬組織內(nèi)的Aβ_(1-40)和Aβ_(1-42)蛋白的含量,長生固本方、固本健腦液較培元固本膠囊作用佳。4.長生固本方、固本健腦液及培元固本膠囊均能促進新生神經(jīng)元的增值和成熟,并且無明顯差異,能有效改善癡呆癥狀。
