【摘要】：目的通過本次臨床研究,評價藥物貼敷配合灸法治療頸椎病(頸型)頸痛的臨床療效,為藥物貼敷配合灸法治療頸椎病(頸型)頸痛的臨床推廣提供依據(jù)。方法以患者到醫(yī)院時分配的就診號為依據(jù),按照隨機(jī)的原則將90例合格的患者平均分為兩組,其中一組為對照組,該組患者只接受針刺治療,另一組為治療組,該組患者接受藥物貼敷配合艾灸治療。治療方法:對照組使用單純針刺法,選取的穴位分別為夾脊、風(fēng)池、后溪、合谷、天窗、阿是穴,一天治療一次,連續(xù)治療5次后停止。治療組使用藥物貼敷配合灸法,治療部位以壓痛點(diǎn)以及有條索狀或點(diǎn)狀結(jié)節(jié)的患處為主,一天治療一次,連續(xù)治療5次后停止。兩組在山西中醫(yī)學(xué)院第三中醫(yī)院針灸三科老師的指導(dǎo)下常規(guī)治療。采用NPQ頸痛評分量表、健康狀況調(diào)查問卷SF-36、McGill評分量表三個量表對兩組分別進(jìn)行客觀數(shù)據(jù)的記錄,并對記錄的數(shù)據(jù)進(jìn)行統(tǒng)計學(xué)分析,得到試驗(yàn)的研究結(jié)論,研究探討藥物貼敷配合灸法治療頸椎病(頸型)頸痛是否有確切的療效。結(jié)果1.NPQ頸痛評分量表評分比較:治療前,兩組患者的NPQ量表評分結(jié)果基本保持一致,無顯著性差異(P0.05)。治療前后評分的組內(nèi)對比:組內(nèi)比較,治療組和對照組治療前后評分差異明顯,具有非常顯著性差異(P0.01),組間比較,兩組有顯著性差異(P0.05),治療組優(yōu)于對照組。2.簡式McGill疼痛問卷評分比較:尚未接受治療前,治療組和對照組比較,差異無統(tǒng)計學(xué)意義(P0.05)。治療后,治療組和對照組組內(nèi)比較,都有非常顯著性差異(P0.01),所有患者接受治療后,兩組之間進(jìn)行對比分析,有顯著性差異(P0.05),治療組優(yōu)于對照組。3.SF-36生存質(zhì)量評分比較:治療前SF-36生存質(zhì)量8維度評分組間比較評分相近,不具有顯著性差異(P0.05)。治療前后比較,治療組和對照組組內(nèi)差異都有顯著意義(P0.05)。治療后,兩組組間比較,在社會功能維度和生理機(jī)能維度,兩組差異有顯著意義(P0.05)。兩組在生理職能維度、軀體疼痛維度和精力維度有非常顯著意義(P0.01)。在健康狀況、情感職能、精神健康三個維度方面(P0.05),組間對比不具有顯著性差異。治療組優(yōu)于對照組。4.所有患者接受完治療后,統(tǒng)計總有效率然后進(jìn)行組間對比,結(jié)果顯示有統(tǒng)計學(xué)意義(P0.01)。治療組的療效優(yōu)于對照組。結(jié)論:1.研究初步表明,兩種療法對治療頸椎病(頸型)頸痛均有較好臨床療效。2.研究初步表明,藥物貼敷配合灸法治療頸椎病頸痛療效優(yōu)于單純針刺組,且治療組具有舒適無痛苦、無創(chuàng)傷、操作方法簡便、易于推廣的優(yōu)勢。
[Abstract]:Objective to evaluate the clinical efficacy of drug application combined with moxibustion in the treatment of cervical spondylopathy (cervical type) by this clinical study. To provide the basis for the clinical popularization of drug application combined with moxibustion in the treatment of cervical spondylopathy (cervical type). According to the random principle, 90 eligible patients were divided into two groups, one group was the control group, the other group was the treatment group. The patients in this group were treated with drug application combined with moxibustion. The control group was treated with simple acupuncture. The selected points were Jiaji, Fengchi, Houxi, Hegu, skylight, Ashi, once a day. The treatment group was treated once a day with tenderness points and the affected areas with striped or punctate nodules. The two groups were treated regularly under the guidance of the teachers of acupuncture and moxibustion in the third Chinese Medicine Hospital of Shanxi College of traditional Chinese Medicine. The NPQ neck pain scale and the health status questionnaire SF-36 were used. The objective data of the two groups were recorded by three scales of McGill rating scale, and the recorded data were statistically analyzed, and the conclusion of the experiment was obtained. To investigate whether there is a definite curative effect on cervical pain of cervical spondylosis (cervical type) treated by drug application combined with moxibustion. 1. Comparison of NPQ score scale: before treatment. There was no significant difference in the scores of NPQ scale between the two groups before and after treatment (P 0.05). There was a significant difference between the treatment group and the control group before and after treatment. There was a very significant difference between the two groups (P0.01, P0.05). The treatment group is better than the control group .2.The simple McGill pain questionnaire score comparison: before the treatment, the treatment group and the control group, the difference is not statistically significant after treatment. Compared with the control group, there is a very significant difference between the treatment group and the control group, all patients received treatment, the two groups were compared and analyzed, there was significant difference (P 0.05). The quality of life of the treatment group was better than that of the control group. 3. Comparison of the scores of quality of life of the treatment group: before treatment, the scores of SF-36 quality of life 8 dimension scores were similar between the groups. There was no significant difference before and after treatment, there were significant differences between the treatment group and the control group. After treatment, there was a significant difference between the two groups. In the social function dimension and physiological function dimension, there was significant difference between the two groups in the physiological function dimension. The physical pain dimension and the energy dimension have very significant significance P0.01. in the health condition, the affective function, the mental health three dimensions P0.05). There was no significant difference between the two groups. The treatment group was superior to the control group. 4. After all the patients received the treatment, the total effective rate was counted and then compared between the groups. The results showed that there was significant difference between the two groups. The curative effect of the treatment group was better than that of the control group. Two kinds of treatment for cervical spondylopathy (cervical type) cervical pain have good clinical efficacy. 2. The preliminary study showed that the drug application combined with moxibustion treatment of cervical Spondylotic cervical pain is better than simple acupuncture group. The treatment group has the advantages of comfortable, no pain, no trauma, simple operation and easy to be popularized.
