【摘要】:目的:通過(guò)實(shí)驗(yàn)研究觀察芪黃保腎顆粒對(duì)單側(cè)輸尿管結(jié)扎術(shù)導(dǎo)致腎衰竭幼鼠腎小管上皮細(xì)胞E-cadherin的表達(dá),旨在探討芪黃保腎顆粒在延緩兒童腎衰竭中的作用機(jī)理。方法:將75只體重80-120g,4周齡的SD雄性幼鼠隨機(jī)分為5組:假手術(shù)組、模型組、芪黃保腎顆粒低、中、高劑量組(簡(jiǎn)稱(chēng)低、中、高劑量組),采用單側(cè)輸尿管結(jié)扎(UUO)法建立腎衰竭幼鼠模型,術(shù)前一天12h禁食,飲水自由,術(shù)后即開(kāi)始給藥灌胃。假手術(shù)組、模型組僅給予蒸餾水進(jìn)行灌胃,低、中、高劑量組分別按人與動(dòng)物等效劑量換算法,給予幼鼠芪黃保腎顆粒的用藥劑量分別是2g/kg/d、4g/kg/d、6g/kg/d進(jìn)行灌胃,一次1ml,每日早8:00到晚18:00兩次灌胃,連續(xù)21天。在造模后的第21天選擇每組15只幼鼠收集血液,尿液及腎臟組織后處死,每只幼鼠采取24h留尿及腹主動(dòng)脈采血法,每次采血2-3ml,檢測(cè)血肌酐(Scr)、血清尿素氮(BUN)、血清白蛋白(AIb)及24尿蛋白定量(24Pr),采血后迅速摘下梗阻側(cè)腎臟,觀察其外觀,剝?nèi)ネ饽?生理鹽水沖洗干凈后,將腎臟組織浸于9%福爾馬林藥液中充分固化。通過(guò)模型幼鼠腎組織HE染色、免疫組化法觀察腎臟病理形態(tài)學(xué)的變化以檢測(cè)造模是否成功,同時(shí)以檢測(cè)腎組織中E-cadherin的含量,觀察芪黃保腎顆粒對(duì)E-cadherin表達(dá)的影響,用以確定芪黃保腎顆粒是否通過(guò)抑制E-cadherin的黏附過(guò)程實(shí)現(xiàn)延緩TEC損傷進(jìn)展的。結(jié)果:1.與模型組相比較,芪黃保腎顆粒各劑量組幼鼠梗阻側(cè)腎臟腫大、積水均較輕,幼鼠一般狀態(tài)較好,腎衰癥狀及體征優(yōu)于模型組。2.與模型組相比較,芪黃保腎顆粒各劑量組幼鼠的Scr、BUN均低于模型組(P0.05),芪黃保腎顆粒中劑量組幼鼠Scr、BUN水平最低,而中劑量組與高劑量組Scr、BUN水平相當(dāng),組間比較沒(méi)有統(tǒng)計(jì)學(xué)意義(P0.05)。同時(shí)芪黃保腎顆粒組可以提高腎衰幼鼠AIb。3.與模型組比較,免疫組化半定量分析顯示腎小管上皮細(xì)胞E-cadherin的表達(dá)均高于模型組。其中芪黃保腎顆粒中劑量組幼鼠腎小管上皮細(xì)胞E-cadherin表達(dá)最高(P0.05)。結(jié)論:1.芪黃保腎顆粒能明顯改善腎衰竭幼鼠的腎臟病理?yè)p害,調(diào)整機(jī)體整體情況,緩解腎衰竭幼鼠的癥狀,同時(shí)延緩腎功能的進(jìn)展。2.芪黃保腎顆粒能夠上調(diào)腎衰幼鼠腎臟組織E-cadherin的表達(dá),減少腎小管基底膜的破壞,使細(xì)胞外基質(zhì)成分合成減少,從而抑制腎間質(zhì)纖維化的發(fā)展。3.芪黃保腎顆粒各劑量組在一定程度上均能保護(hù)腎衰幼鼠的腎功能,降低腎小管的損害和拮抗腎間質(zhì)纖維化的發(fā)生,其中芪黃保腎顆粒中劑量的效果最佳,為治療兒童腎衰竭提供了臨床依據(jù)。
[Abstract]:Objective: to observe the expression of E-cadherin in renal tubular epithelial cells of rats with renal failure induced by unilateral ureteral ligation. Objective: to investigate the mechanism of Qihuang Baoshen granule in delaying children renal failure. Methods: 75 male SD rats weighing 80-120g / 4 weeks were randomly divided into five groups: sham operation group and model group. Qi Huang Bao Shen granule is low, medium, high dose group (referred to as low, medium, high dose group), using UUO method of unilateral ureteral ligation to establish the model of kidney failure young rats, fasting 12 hours before operation, drinking water freedom. The model group was only given distilled water, the low, middle and high dose groups were converted according to the equivalent dose of human and animal. The dosage of Qihuangbaoshen granule was 2 g / kg / d 4 g / kg / d and 6 g / kg / d respectively. From 8:00 to 18:00, the rats were given orally twice daily for 21 days. 15 young rats in each group were selected to collect blood, urine and kidney tissue and were killed after 21 days. Blood samples were collected from 24 hours urine and abdominal aorta of each young rat. Blood creatinine (SCR) and serum urea nitrogen (bun) were detected. Serum albumin (AIb) and 24 urinary protein (24) were quickly removed from the obstructed kidney after blood collection, the appearance of the kidney was observed, the outer membrane was peeled off, and the saline was washed out. The kidney tissue was fully solidified in 9% formalin solution. The pathological changes of kidney were observed by HE staining and immunohistochemical method in order to detect the success of the model. At the same time, to detect the content of E-cadherin in renal tissue, to observe the effect of astragalus baoshen granule on E-cadherin expression. To determine whether astragalus baoshen granules can delay the progress of TEC damage by inhibiting the adhesion of E-cadherin. Results: 1. Compared with the model group. Qihuangbaoshen granule of each dose group of young rats obstruction side kidney swelling, hydronephrosis are light, the general state of young rats is better, renal failure symptoms and signs than model group. 2. Compared with model group. 2. The levels of Scr-bun in each dose group of Qihuangbaoshen granules were lower than those of model group (P0.05A), and the level of Scr-bun was the lowest in the middle dose group of Qihuangbaoshen granule. The level of Scrn bun in middle dose group was similar to that in high dose group. There was no significant difference between the two groups (P 0.05). At the same time, Astragalus baoshen granule group could increase the AIb.3. compared with the model group. The expression of E-cadherin in renal tubular epithelial cells was significantly higher than that in the model group, and the E-cadherin table of renal tubular epithelial cells in the middle dose group of Qihuangbaoshen granules was higher than that in the model group. The highest (. Conclusion 1. Astragalus baoshen granule can obviously improve renal pathological damage in young rats with renal failure. Adjust the overall situation of the body, alleviate the symptoms of young rats with renal failure, and delay the progress of renal function. 2. Qihuangbaoshen granule can up-regulate the expression of E-cadherin in kidney tissue of young rats with renal failure. Reduce the destruction of renal tubule basement membrane, reduce the synthesis of extracellular matrix components, so as to inhibit the development of renal interstitial fibrosis .3.Astragalus baoshen granule can protect the renal function of young rats to a certain extent. It can reduce the damage of renal tubules and antagonize the occurrence of renal interstitial fibrosis, of which the middle dose of Qihuangbaoshen granule is the best, which provides the clinical basis for the treatment of children with renal failure.
