【摘要】：研究目的:總結(jié)進(jìn)行性斑狀色素減少癥的臨床表現(xiàn),并運(yùn)用反射式共聚焦激光顯微鏡輔助進(jìn)行性斑狀色素減少癥的診斷,總結(jié)歸納進(jìn)行性斑狀色素減少癥皮損在反射式激光共聚焦顯微鏡下的圖像特征。分析進(jìn)行性斑狀色素減少癥患者的中醫(yī)體質(zhì)類型,探索可能發(fā)病因素與中醫(yī)體質(zhì)之間是否存在科學(xué)規(guī)律,從而充實(shí)進(jìn)行性斑狀色素減少癥的中醫(yī)方面內(nèi)容,為疾病的中醫(yī)藥診斷治療提供基礎(chǔ)。研究方法:本研究共納入30例符合進(jìn)行性斑狀色素減少癥診斷標(biāo)準(zhǔn)的患者。對(duì)30例納入的進(jìn)行性斑狀色素減少癥患者進(jìn)行真菌鏡檢、伍氏燈和共聚焦掃描顯微鏡檢查輔助診斷。登記患者一般情況、現(xiàn)病史資料,填寫中醫(yī)體質(zhì)分類表和皮膚病生活質(zhì)量調(diào)查表,對(duì)所有皮損進(jìn)行拍照。研究結(jié)果:進(jìn)行性斑狀色素減少癥好發(fā)于青年人,男女未見差異。圓形或者橢圓形淡白斑沿身體正中線兩側(cè)成網(wǎng)狀分布,背部、腹部與腰部是發(fā)病率最高的幾個(gè)部位。IV皮膚,皮膚油膩,出汗多,有既往痤瘡史的患者多見。伍氏燈皮損區(qū)下均可見點(diǎn)狀紅色熒光,紅色熒光密度低于自身痤瘡皮損區(qū)。進(jìn)行性斑狀色素減少癥在RCM下表現(xiàn)具有特異性:基底細(xì)胞環(huán)大致完整,色素減退。研究結(jié)論:RCM能輔助診斷進(jìn)行性斑狀色素減少癥,在RCM下與白癜風(fēng)易鑒別。進(jìn)行性斑狀色素減少癥對(duì)患者的影響輕微。進(jìn)行性斑狀色素減少癥患者中醫(yī)體質(zhì)類型主要見于濕熱質(zhì)、氣虛質(zhì)、和陰虛質(zhì)。進(jìn)行性斑狀色素減少癥患者中醫(yī)體質(zhì)類型的分布與年齡、性別、運(yùn)動(dòng)量無關(guān)聯(lián)。進(jìn)行性斑狀色素減少癥患者中醫(yī)體質(zhì)類型的分布與患者出汗量、膚色深淺有關(guān)聯(lián)。
[Abstract]:Objective: To summarize the clinical manifestations of progressive macular hypomelanosis, and using a reflective confocal laser microscope assisted progressive macular hypomelanosis diagnosis, summarized macular pigment reduced image feature in reflective laser confocal microscope in lesions. Analysis of macular pigment reduction in patients with TCM type of constitution and to explore the possible existence of scientific laws between risk factors and TCM Constitution, so as to enrich of macular pigment reduce TCM aspects of disease, and provide basis for Chinese medicine diagnosis and treatment of diseases. Methods: This study included 30 patients with progressive macular hypomelanosis patients. The diagnostic criteria of 30 cases included the progressive macular hypomelanosis patients with microscopic fungi, Wu light wood and confocal scanning microscopy examination diagnosis. The general condition of the patient registration, now Medical records, fill in the constitution of TCM classification and dermatology quality of life questionnaire, all lesions were pictures. Results: progressive macular hypomelanosis occurs in young men and women, there was no difference. Round or oval pale white spots along the body midline on both sides of network distribution, back, abdomen and waist is the incidence of several parts the highest.IV skin, oily skin, sweating, a previous history of acne patients see more. Both punctate red fluorescence under the lamp of five family lesions, red fluorescence density lower than their acne lesions area. For macular pigment reduce symptoms with specificity in RCM: basal cell ring largely intact, hypopigmentation. Conclusion: RCM can aid in the diagnosis of progressive macular hypomelanosis, RCM in vitiligo and easy to identify. Progressive macular hypomelanosis on patients with mild influence. The macular pigment reduction In patients with TCM type of constitution mainly in the damp heat, Qi deficiency, and yin deficiency. Macular pigment reduce distribution and age, TCM type of constitution in patients with gender, no correlation between the amount of exercise. For macular pigment reduction in patients with TCM Constitution Types and the distribution of patients is related to the amount of sweat, skin deep.
