【摘要】:研究背景:隨著社會的發(fā)展,人們的生活水平不斷提高,在飲食和起居等方面有了翻天覆地的變化;飲食的種類及食法具有多樣性,現(xiàn)如今甜、酸、辛味飲食的攝入量表現(xiàn)出日益增高的趨勢,其中辛味飲食不僅受人們喜愛,且攝入量相對較高,辛味飲食對“普胃酶灼”(pho bavi me drod)即“藏醫(yī)火熱學(xué)說”機理有較大的影響。而藏醫(yī)的“普胃酶灼”猶如人體精華之基本、臨床治療之原則、養(yǎng)生保健之方法,使其平衡紊亂,可引起多種疾病,故合理食用辛味飲食等各方面使“普胃酶灼”平衡,是無病者長壽和患病者治療的最佳方法。研究目的:藏醫(yī)辛味飲食對“普胃酶灼”干預(yù)的機理研究為核心目的,且進一步可分為:1.探討辛味食物的性質(zhì)及功能,分析辛味飲食的食量現(xiàn)狀。2.探索藏醫(yī)“普胃酶灼”的特征,制定評定量表。3.挖掘辛味飲食與“普胃酶灼”之間潛在的關(guān)聯(lián)。4.分析按性別等基本信息與辛味飲食攝入量和“普胃酶灼”有無關(guān)聯(lián)或差異。5.根據(jù)體質(zhì)、年齡、四季等變量不同,擬定如何合理飲食的指導(dǎo)方案。研究方法:文獻研究:首先藏醫(yī)藥經(jīng)典書籍中,搜集和分析辛味食藥的性質(zhì),適食的益處與過食的危害。其次,基于藏醫(yī)對“普哇”(pho ba)的定義、結(jié)構(gòu)進行分析,且整理和歸納與“普胃酶灼”有關(guān)的癥狀,以出現(xiàn)頻率選取具有代表性癥狀制定其評定量表。最后,在單味藥物和組方中搜集和匯總對“普胃酶灼”有益的食藥。從而分析辛味飲食與“普胃酶灼”之間的關(guān)系制定合理的調(diào)查問卷。調(diào)查研究:已制定的問卷經(jīng)專家和導(dǎo)師修正后,對青海大學(xué)成人教育學(xué)院的學(xué)生隨機(分層抽樣)選取60名學(xué)生進行預(yù)調(diào)查,分析其問卷的合理性,并再次修正后對青海大學(xué)藏醫(yī)學(xué)院的437名學(xué)生以回顧調(diào)查法進行答卷,收集和整理問卷以待分析。分析數(shù)據(jù)方面:1.將收集的數(shù)據(jù)在2013 Microsoft Excel和2013 Microsoft Access中統(tǒng)一和歸納。2.在統(tǒng)計軟件SPSS Statistics 20.0里通過X2檢驗、t檢驗、雙變量相關(guān)分析Speearman、回歸曲線估計等方法對數(shù)據(jù)進行數(shù)據(jù)分析。3.明確所需分析變量之間的統(tǒng)計意義,并對結(jié)果進行全面討論。研究結(jié)果:文獻研究:1.藏醫(yī)辛味主要是由五源中的火源和風(fēng)源,尤其是火源偏盛而成;主要成分為揮發(fā)油;在藏醫(yī)及中醫(yī)所記載的溫性藥物所占比例最高。2.辛味食藥主要醫(yī)治“普胃酶灼”衰弱所誘發(fā)的疾病,如“培根”性疾病,并具有消食開胃,祛除“曲色”(chu ser)、“森”(srin),治療腫瘤、尿潴留、惡瘡等功效;但辛味食藥過量,可引起遺精、暈厥、焦渴等癥狀。3.“普胃酶灼”的最終本質(zhì)為藏醫(yī)三因即:“隆”、“赤巴”、“培根”,其在攝取精華、醫(yī)治疾病、代謝糟粕等各方面都起到了不可缺少的作用。4.以藏醫(yī)古籍為基礎(chǔ)搜集和歸納“普胃酶灼”過盛和衰敗的評定癥候群,其過盛時可出現(xiàn)易消化、體熱、胃痛、口臭、易發(fā)汗且有異味、焦渴、油膩食物致頭痛、眼球發(fā)黃、上吐下瀉偏黃、晚上難眠白天嗜睡、舌苔黃膩、皮膚發(fā)黃等十二個。衰敗時易出現(xiàn)消化不良、飯后不適、胃飽滿感、懶惰、胃脹、腹瀉、體鵼、呃逆及嘔吐、晝夜嗜睡、腸鳴、便秘等十二個。5.在單味藥物和組方中對“普胃酶灼”平衡有益的如蓽撥、小米椒、高良姜、石榴等,組方有六味嘎啦扎散和四味石榴散等。調(diào)查研究:現(xiàn)狀:辛味飲食的攝入量不斷增高,如一天吃一至兩次的樣本占總體的80%,吃微辣或中辣的占85%。同樣,大部分年輕人具有的“普胃酶灼”失衡的癥狀,其中“酶灼”衰弱的癥狀尤為突出。關(guān)聯(lián):隨著辛味飲食攝入量的增加,“普胃酶灼”過盛或衰敗之癥候量表的總值逐漸增高(P0.05);辛味飲食的變量中,常吃麻辣燙類飲食、辛味零食、油炸食品時“普胃酶灼”癥候量表總值依然上升(P0.05);在性別方面,女性的辛味飲食攝入量和“普胃酶灼”異常的量表總值遠高于男性(P0.05)。年齡方面,隨著年齡的增高,辛味飲食攝入量降低,可癥狀增多(P0.05)。另外,不習(xí)慣吃辛辣食物或著重合理飲食者,辛味食物攝入量較低,且“普胃酶灼”衰敗的癥狀也很低(P0.05)。研究結(jié)論:本研究以藏醫(yī)養(yǎng)生保健學(xué)為理論宗旨,以藏醫(yī)飲食療法、起居療法、藏醫(yī)藥理學(xué)和病機學(xué)等為基礎(chǔ),結(jié)合現(xiàn)代問卷調(diào)查和數(shù)據(jù)統(tǒng)計方法,共同分析后闡明辛味飲食對“普胃酶灼”干預(yù)的機理;其對如今提倡治未病,呼吁食療的現(xiàn)狀具有一定的指導(dǎo)和推進意義,并對辛味飲食和“普胃酶灼”等宏觀學(xué)說進行歸類和分層,且擬定評定量表;使飲食的攝入量和“酶灼”的盛衰得意量化。另在藏醫(yī)古籍中搜集了常用食物之間的生克關(guān)系,并整理了對“普胃酶灼”有益的經(jīng)典藥方,使食藥同用、主于食療;身病同調(diào)、主于調(diào)身,從而對實踐藏醫(yī)食療法、研究藏醫(yī)“酶灼”學(xué)、探索藏藥六味學(xué)、完善藏醫(yī)養(yǎng)生學(xué)等方面皆有較好的意義。
