【摘要】：目的:隨著現(xiàn)代生活節(jié)奏加快,經濟的快速增長,個體對于社會生活的多種應激、變化不能良好適應,使得心身疾病在現(xiàn)代疾病譜上不斷凸顯。這種心身疾病,對個體體能狀態(tài)上具有十分大的危害,不僅僅會嚴重困擾個體的生活,而且增加了其他疾病治療的復雜程度。目前越來越多的臨床工作者認為社會心理因素對于心身疾病的發(fā)生發(fā)展,有著極為重要的致病作用。目前現(xiàn)代醫(yī)學以生物-心理-社會醫(yī)學模式作為指導,對心身疾病已經有了深刻的認識,治療方法也逐漸發(fā)展日益多元。然而考慮實際臨床工作中心身疾病具有較高復發(fā)性,難治愈,在病程中易復燃或復發(fā)。故而從祖國醫(yī)學角度出發(fā),挖掘名老中醫(yī)診療經驗,為心身疾病患者積極尋找其他安全、有效的治療方法,提高患者生活質量,具有重要臨床意義。戰(zhàn)麗彬教授在多年的臨床實踐過程中,結合經典論著,對郁證及共病郁證的診療思路獨到,治療效果顯著。故本研究通過系統(tǒng)收集2013-2015年戰(zhàn)麗彬教授門診臨床醫(yī)案,歸納分析她診治情志病的學術思想、經驗及臨床用藥規(guī)律,從而以中醫(yī)藥治療手段為著眼,為心身疾病的診治尋找新的突破。此外本文通過對戰(zhàn)麗彬教授診治郁證及共病郁證進行探討,對于繼承和發(fā)揚名老中醫(yī)的學術思想也具有十分重要意義。方法:本研究主要運用兩種研究方法:文獻采集,臨床醫(yī)案分析。具體內容分為兩個部分:第一部分,通過臨床侍診,收集、整理、研習戰(zhàn)麗彬教授大量醫(yī)案,并結合教授的悉心指導,對戰(zhàn)麗彬教授治療郁證的治學經驗及臨床學術思想進行分析、總結。第二部分,戰(zhàn)麗彬教授在多年臨床實踐中對情志致病有深刻體會。并從共病郁證角度出發(fā),極具創(chuàng)新的認識和理解了臨床中婦科疾病、腸易激綜合征、高脂血癥等臨床常見疾病。故本部分內容,則重點從戰(zhàn)麗彬教授特色鮮明的治學思想及臨床經驗出發(fā),對戰(zhàn)麗彬教授共病郁證的臨床用藥經驗及防治方法進行概括總結。第三部分,利用傳統(tǒng)的醫(yī)案分析方法,選取典型郁證-呃逆、經斷前后諸癥共病郁證、痹癥共病郁證三病進行舉例分析,從而將戰(zhàn)麗彬教授從治學經驗、臨床學術思想到臨床效果進行全面總結。結果:第一部分:戰(zhàn)麗彬教授發(fā)展了郁證的理論思想,較早的提出診治郁證的“從中醫(yī)體質學說角度,論治人群郁證易感性”、“從形神合一角度,論郁證與臟腑功能的雙向影響”、“基于臟腑理論,從肝脾同調角度,論治郁證”三方面學術思想。第二、三部分:戰(zhàn)麗彬教授治療臨床常見疾病共病郁證的體會。戰(zhàn)麗彬教授中西醫(yī)匯通,創(chuàng)新的將現(xiàn)代醫(yī)學對心身疾病的認識及我國先賢學術思想相融合,結合自身多年臨床實踐,從共病郁證角度出發(fā),認識和理解了婦科疾病、腸易激綜合征、高脂血癥的發(fā)生、發(fā)展過程及防治,形成極具個人特色的臨床診療經驗。并通過選取經典醫(yī)案具體闡述其對郁證-呃逆、經斷前后諸癥共病郁證、痹癥共病郁證的臨床辨證論治思路和診療方案。結論:戰(zhàn)麗彬教授常從中醫(yī)古籍汲取營養(yǎng),學貫古今,且學古而不泥古,作為一名中西醫(yī)匯通的現(xiàn)代中西醫(yī)結合專家,她常將現(xiàn)代醫(yī)學對診治疾病的認識,同古代先賢的學術思想巧妙融合,并結合當今社會所特有人文、經濟特點,在臨床實踐過程中,積累了豐富的經驗。尤其對于郁證及共病郁證的認識和治療上,更形成了完善的理法方藥體系,并在臨床上驗證,取得了顯著的治療效果,其學術思想和經驗有著較強的臨床指導作用,為臨床中醫(yī)藥治療心身疾病提供了新思路和新手段。本文第二、三部分的醫(yī)案分析則是戰(zhàn)麗彬教授的新穎學術思想、及其實際應用于臨床可獲得顯著療效的具體而強有力的證明。
[Abstract]:Objective: with the accelerated pace of modern life, the rapid growth of the economy, the individual stress for a variety of social life, can not adapt well to the changes, psychosomatic diseases continue to highlight in the modern disease spectrum. This has very big psychosomatic diseases, the harm to the individual physical condition, will not only seriously troubled individual life, but also increase the complexity of the treatment of other diseases. At present, more and more clinical workers believe that social and psychological factors for the occurrence and development of psychosomatic diseases, has a very vital role in the development of modern medicine. The bio psycho social medical model as a guide to psychosomatic diseases has been profound understanding, treatment is also gradually increasing multiple however. Considering the actual clinical work center disease with high recurrent, difficult to cure, easy to relapse or recurrence in the course of the disease. Therefore, from the perspective of traditional Chinese Medicine Hair, mining experience of famous Chinese medicine diagnosis and treatment, for patients with psychosomatic diseases actively looking for other safe and effective treatment, improve the quality of life of patients, has important clinical significance. Professor Zhan Libin in clinical practice for many years, combined with the classic theory, diagnosis and treatment of depression and comorbidity of depressive syndrome and unique treatment effect. In this study, through the systematic collection of clinical medical professor Zhan Libin outpatient 2013-2015, diagnosis and treatment of emotional disease inductive analysis of her academic thought, law of experience and clinical medication, so as to Chinese medicine treatment for eyes, looking for a new breakthrough for the diagnosis and treatment of heart disease. In addition this paper discusses professor Zhan Libin on the treatment of melancholia and comorbidity melancholia, also has very important significance to inherit and carry forward the name of the old Chinese medicine academic thought. Methods: This study used two research methods: literature collection, clinical case analysis Specific content is divided into two parts: the first part, through the clinical diagnosis of paternity, collection, collation, research professor Zhan Libin a large number of medical records, and with the guidance of Professor, Professor Zhan Libin thought on treating depression with teaching experience and clinical academic analysis summary. The second part, Professor Zhan Libin have a deep understanding of emotional diseases in many years of clinical practice. Starting from the comorbidity of depression angle, innovative understanding and understanding of the clinical gynecological disease, irritable bowel syndrome, hyperlipidemia and other common clinical diseases. This part is mainly from the academic thought and clinical experience of Professor Zhan Libin's distinctive features of clinical experience method of prevention and cure of Professor Zhan Libin comorbid depression syndrome were summarized. The third part, using the method of analysis of traditional medical records, typical depression - hiccup, menopausal disorder comorbid depression syndrome, arthralgia comorbidity For example three depression disease, and Professor Zhan Libin from the teaching experience, clinical academic thoughts to clinical effect was comprehensively summarized. Results: the first part: Professor Zhan Libin developed the theory of thought of melancholia, earlier proposed diagnosis and treatment of depression syndrome "from the perspective of the Physique Theory of Chinese medicine, on the treatment of depression syndrome susceptibility," the crowd "from the perspective of one spirit, mutual influence" theory of melancholia and viscera function, based on the theory of internal organs, liver and spleen from the perspective of coherence, the three aspects of the academic thought of treating depression syndrome ". The second, third part: the experience of Professor Zhan Libin in the treatment of depression in clinical common disease. Professor Zhan Libin, the confluence of Chinese and Western medicine, the combination of innovation understanding of modern medicine on psychosomatic diseases and our fathers academic thoughts, combined with their years of clinical practice, starting from the comorbidity of depression angle, the recognition and understanding of the gynecological disease, irritable bowel syndrome, hyperlipidemia Occurrence, development process and prevention, clinical experience form very personal characteristics. And through the selection of classic cases illustrate its hiccup of depression syndrome, menopausal disorder comorbid depression syndrome, the treatment and treatment of arthralgia syndrome clinical dialectical thinking the comorbidity of depression syndrome. Conclusion: Professor Zhan Libin often draw nutrition from ancient Chinese medicine, science and science through ancient and modern, ancient and not imitation, as a modern western medicine Huitong combining traditional Chinese and Western medicine expert, she often will know modern medicine for diagnosis and treatment of the disease, with the ancient sages of the academic thought of fusion, and combining with the current social, cultural and economic characteristics, in clinical practice, has accumulated rich experience. Especially the cognition and treatment for depression and comorbid depression syndrome, also formed the perfect prescription system, and verified in clinical practice, has achieved remarkable curative effect, the academic thought and experience are strong Clinical guidance, provides new ideas and new methods for clinical medicine in the treatment of psychosomatic diseases. Case analysis of the second, third part is the new academic thought of Professor Zhan Libin, and its clinical application can obtain significant effect of specific and powerful proof.
