【摘要】：研究目的以江蘇省中醫(yī)院舒鵬教授臨床治療胃癌的實(shí)踐經(jīng)驗(yàn)為基礎(chǔ),以數(shù)據(jù)挖掘技術(shù)為手段,對(duì)舒鵬教授近年來(lái)辨治胃癌的臨床經(jīng)驗(yàn)及學(xué)術(shù)思想進(jìn)行歸納、探討和分析,并進(jìn)一步總結(jié)舒鵬教授治療胃癌的經(jīng)驗(yàn)及學(xué)術(shù)思想,以供同道借鑒,為提高中醫(yī)藥在胃癌治療中的療效做出努力。研究方法對(duì)舒鵬教授2012年1月至2015年1月門診診治的胃癌病案進(jìn)行整理和收集,嚴(yán)格按照納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn)進(jìn)行篩選,將符合標(biāo)準(zhǔn)的162例胃癌病案資料建立臨床病案數(shù)據(jù)庫(kù),運(yùn)用頻數(shù)分布、聚類分析等方法著重挖掘病案中癥狀、辨證分型、方藥之間的規(guī)律,結(jié)合舒鵬教授的臨證經(jīng)驗(yàn)及學(xué)術(shù)思想,對(duì)挖掘結(jié)果進(jìn)行總結(jié)、歸納和分析。研究結(jié)果在舒鵬教授診治的這162例胃癌患者中,男性107例,女性55例,年齡在25-85歲之間,平均年齡60±1.09歲。所有診次癥狀中,頻數(shù)芝10的有13種,分別為乏力(59例,36.42%)、納差(54例,33.33%)、噯氣(31例,19.14%)、便溏(28例,17.28%)、泛酸(24例,14.81%)、畏寒(23例,14.20%)、便秘(12例,7.41%)、胃脹(16例,9.88%)、進(jìn)食梗阻(13例,8.02%)、嘈雜(13例,8.02%)、嘔吐(12例,7.41%)、失眠(11例,6.79%)、胃痛(10例,6.17%)。采用將中醫(yī)證型規(guī)范成單證的方式,將單證按照虛證和實(shí)證進(jìn)行分類,結(jié)果表明虛證中常見(jiàn)證型為脾胃氣虛(54.32%)、脾胃虛寒(26.54%)及氣陰兩虛(19.14%)、胃陰虛(17.28%),而癌毒內(nèi)聚(27.16%)、胃氣郁滯(24.07%)及寒濕困脾(18.52%)、肝脾不調(diào)(12.35%)是主要的實(shí)證。舒鵬教授選用的中藥共18類,其中頻數(shù)在100次以上,頻率在3%上的藥物僅8類,累計(jì)頻率占88.43%,分別為消食藥、補(bǔ)虛藥、化痰止咳平喘藥、活血化瘀藥、行氣藥、清熱藥、收澀藥及溫里藥。舒鵬教授治療胃癌使用藥比較廣泛,共使用中藥152種,總計(jì)3354昧次,頻率70%也僅有13味中藥：炙甘草(98.15%)、炒稻芽(93.83%)、炒麥芽(93.83%)、黃芪(90.74%)、山楂(90.74%)、神曲(90.12%)、雞內(nèi)金(87.04%)、白術(shù)(82.72%)、陳皮(77.16%)、法半夏(76.54%)、黨參(72.22%)、浙貝母(70.37%)。研究結(jié)論通過(guò)研究發(fā)現(xiàn),胃癌的臨床常見(jiàn)癥狀：為乏力、納差、便溏、便秘、胃脹、失眠、畏寒、進(jìn)食梗阻、噯氣、泛酸、嘈雜、胃痛及嘔吐。常見(jiàn)證型以脾胃氣虛、癌毒內(nèi)聚、脾胃虛寒、胃氣郁滯及氣陰兩虛、寒濕困脾、胃陰虛、肝脾不調(diào)為主。舒鵬教授治療胃癌常用藥物補(bǔ)虛藥、消食藥、化痰止咳平喘藥、行氣藥、活血化瘀藥、清熱藥、收澀藥及溫里藥,如炙甘草、炒稻芽、炒麥芽、黃芪、山楂、神曲、雞內(nèi)金、白術(shù)、陳皮、法半夏、黨參、浙貝母等�？傮w治法為健脾養(yǎng)胃,扶正固本為主,祛邪為輔。
