【摘要】：目的:本文旨在研究西黃膠囊對(duì)于粉刺性乳癰患者的治療作用,探尋西黃膠囊治療粉刺性乳癰的療效和藥物作用的途徑,為西黃膠囊治療粉刺性乳癰提供臨床和科研的基礎(chǔ)。方法:收集2015年3月至2015年10月河南省中醫(yī)院乳腺科門(mén)診初收治的粉刺性乳癰患60例。所納入的患者絕大部分有乳腺腫塊、乳腺紅腫熱痛以及膿腫形成,部分已形成瘺管或竇道。西黃膠囊出自西黃丸,其功效為清熱解毒、化痰散結(jié)、活血化瘀、消腫止痛,組成成分為牛黃、麝香、乳香和沒(méi)藥。本研究通過(guò)觀察粉刺性乳癰患者的臨床表現(xiàn)和血液的檢查結(jié)果,給予口服西黃膠囊治療,記錄治療過(guò)程中的臨床表現(xiàn),血細(xì)胞分析、CRP及性激素六項(xiàng)等指標(biāo)的變化;并對(duì)入組的患者進(jìn)行定期隨訪,運(yùn)用SPSS18.0軟件進(jìn)行統(tǒng)計(jì)分析并評(píng)價(jià)。結(jié)果:(1)應(yīng)用西黃膠囊為主治療粉刺性乳癰患者2周、4周后總有效率分別為43.33%、93.33%,乳房腫塊治療前后明顯縮小,皮膚紅腫癥狀亦緩解明顯,經(jīng)統(tǒng)計(jì)學(xué)分析有顯著差異,P0.01。(2)治療后WBC、NE%、CRP計(jì)數(shù)下降,均有統(tǒng)計(jì)學(xué)意義P0.05。(3)治療后性激素六項(xiàng)指標(biāo)與治療前相比較有不同程度的下降,但雌激素、孕激素、睪酮、促黃體生成素、促卵泡生成素及泌乳素的指標(biāo)同治療前相比較,均無(wú)統(tǒng)計(jì)學(xué)意義,即P0.05。結(jié)論:(1)西黃膠囊治療粉刺性乳癰的療效肯定,值得臨床推廣和應(yīng)用。(2)西黃膠囊治療粉刺性乳癰可能與其抗炎功效有關(guān)。(3)調(diào)節(jié)內(nèi)分泌功能可能是西黃膠囊治療粉刺性乳癰的一個(gè)重要機(jī)理。
[Abstract]:Objective: To study the effect of Xihuang Capsules for the treatment of plasma cell mastitis patients, explore the curative effect and drug effect way Xihuang Capsules in the treatment of acne of acute mastitis, provide the basis for clinical and scientific research for Xihuang Capsules in the treatment of plasma cell mastitis. Methods: from March 2015 to October 2015 in Henan Traditional Chinese Medicine Hospital outpatient breast early treated 60 patients with plasma cell mastitis. Patients with most breast lumps, breast swelling and heat pain and abscess formation, part has formed a fistula or sinus. From Xihuang Pill Xihuang Capsules, the effect of heat clearing and detoxifying, phlegm, blood stasis, detumescence and acesodyne, composition of bezoar, musk, frankincense and myrrh. This research through clinical observation performance and blood plasma cell mastitis patients to check the results, given oral Xihuang Capsules treatment, record the clinical manifestations in the course of treatment, blood cell analysis, CRP The changes of sex hormones and six other indicators; and in the group of patients were regularly followed up for statistical analysis and evaluation by SPSS18.0 software. Results: (1) the application of Xihuang Capsules treatment of plasma cell mastitis in 2 weeks, 4 weeks after the total efficiency were 43.33%, 93.33%, breast lumps before and after treatment was significantly reduced also, the symptoms of skin irritation was relieved, the statistical analysis had significant differences, P0.01. (2) after treatment, WBC, NE%, CRP counts decreased, had statistical significance (3 P0.05.) after the treatment of sex hormone six indicators compared with before treatment were decreased, but different degree of estrogen, progesterone, testosterone, luteinizing in the follicle stimulating hormone index and lactation compared with before treatment, were not statistically significant, P0.05. (1) conclusion: the curative effect of Xihuang Capsules in the treatment of plasma cell mastitis, is worthy of promotion and application. (2) xihunag capsule in the treatment of acne lotion Carbuncle may be related to anti-inflammatory effect. (3) the regulation of endocrine function may be an important mechanism of Xihuang capsule in the treatment of plasma cell mastitis.
