【摘要】：目的:本課題以舒筋通絡(luò),解痙止痛,整復(fù)錯(cuò)位為原則,運(yùn)用針刺結(jié)合推拿治療少年兒童寰樞關(guān)節(jié)半脫位,通過觀察患兒臨床癥狀、Northwick Park頸痛量表(NPQ)評分,客觀評價(jià)針刺結(jié)合推拿治療少年兒童寰樞關(guān)節(jié)半脫位的臨床療效,為針刺結(jié)合推拿療法治療本病了提供一種療效確切、操作安全的技術(shù)。方法:選取符合納入標(biāo)準(zhǔn)的64例寰樞關(guān)節(jié)半脫位患兒,按1:1比例隨機(jī)分為2組,針刺結(jié)合推拿組為治療組,牽引治療組為對照組,每組均為32例,治療組給予針刺及推拿治療,隔日一次,每周3次,10次為一療程;對照組用坐位枕頜帶間歇牽引,牽引重量為1-3kg,牽引時(shí)間每次20分鐘,隔日一次,每周3次,10次為一個(gè)療程。治療2個(gè)療程,隨訪3個(gè)月。分別于治療2療程后、治療后3個(gè)月后觀察療效。隨時(shí)記錄觀察過程中的不良事件。采用統(tǒng)計(jì)學(xué)軟件SPSS 22.0對收集的數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析。結(jié)果:1.治療前,治療組與對照組的年齡、性別、病程、致病原因等一般情況對比無顯著差異(P0.05),具有可比性。2.治療前,治療組與對照組的NorthwickPark頸痛量表(NPQ)評分比較,差異無顯著性意義(P0.05),兩組具有可比性。3.治療2療程后,治療組與對照組的臨床癥狀、NorthwickPark頸痛量表(NPQ)評分與治療前比較,差異均有顯著性意義(P0.05),提示兩種療法都具有良好的臨床療效。4.治療2療程后,治療組的臨床癥狀、NorthwickPark頸痛量表(NPQ)評分與對照組比較,有顯著性差異(P0.05),提示治療組近期療效優(yōu)于對照組。5.治療結(jié)束后3個(gè)月,治療組臨床癥狀、NorthwickPark頸痛量表(NPQ)評分與對照組的比較,有顯著性差異(P0.05),提示治療組遠(yuǎn)期療效優(yōu)于對照組。結(jié)論:兩種治療方法都能治療少年兒童寰樞關(guān)節(jié)半脫位,但針刺結(jié)合推拿療法在近期及遠(yuǎn)期療效上均優(yōu)于牽引療法,是治療少年兒童寰樞關(guān)節(jié)半脫位的安全有效的方法。
[Abstract]:Objective: to treat atlantoaxial subluxation in children with acupuncture combined with massage according to the principle of relieving muscles and unblocking collaterals, relieving spasmolysis and relieving pain, and treating children with atlantoaxial subluxation, and to observe the clinical symptoms of children with subluxation of atlantoaxial joint. Northwick Park cervical pain scale score, objective evaluation of acupuncture combined with massage treatment of children with atlantoaxial subluxation of the clinical efficacy. Methods: 64 children with atlantoaxial subluxation were selected and randomly divided into 2 groups according to the proportion of 1: 1. Acupuncture combined with massage group as treatment group, traction treatment group as control group, each group were 32 cases, the treatment group was given acupuncture and massage treatment, once every other day, 3 times a week for 10 times as a course of treatment; The control group was treated with intermittent traction of occipitomaxillary band in the sitting position with traction weight of 1-3 kg, traction time of 20 minutes each time, once every other day, 10 times a week for a course of treatment and 2 courses of treatment. The patients were followed up for 3 months after 2 courses of treatment. The therapeutic effect was observed 3 months after treatment. Adverse events during observation were recorded at any time. Statistical software SPSS 22.0 was used to analyze the collected data. Results: 1. Before treatment. There was no significant difference between the treatment group and the control group in age, sex, course of disease, pathogenicity, etc. There was no significant difference between the treatment group and the control group in NorthwickPark cervical pain scale score, the two groups were comparable. 3. After 2 courses of treatment, there was no significant difference between the two groups. The clinical symptoms of the treatment group and the control group were significantly different from those before treatment (P 0.05). After two courses of treatment, the clinical symptoms of the treatment group were compared with those of the control group with the Northwick Park neck pain scale (NPQs). There was significant difference (P 0.05), indicating that the short-term curative effect of the treatment group was better than that of the control group .5.The clinical symptoms of the treatment group were 3 months after the end of treatment. The scores of NorthwickPark cervical pain scale were significantly different from those of the control group (P 0.05). Conclusion: both the two treatment methods can treat atlantoaxial subluxation in children, but acupuncture combined with massage therapy is better than traction therapy in the short and long term. It is a safe and effective method for the treatment of atlantoaxial subluxation in children.
