【摘要】：研究目的:觀察聲帶息肉術(shù)后口服中藥湯劑清咽利喉方療效。通過數(shù)據(jù)分析,評價(jià)清咽利喉方與常規(guī)治療的差別。探索清咽利喉方對聲帶息肉術(shù)后聲帶形態(tài)及功能恢復(fù)的有效性和優(yōu)異性,為聲帶息肉患者術(shù)后的恢復(fù)提供更好療效的治療方案。研究方法:選自2016年1月～2016年12月期間,江蘇省中醫(yī)院耳鼻喉科入院的具有手術(shù)指征、符合診斷標(biāo)準(zhǔn)、入院后行"全麻喉內(nèi)鏡下聲帶息肉摘除術(shù)"的60例病例。入組后予分組,治療組與對照組各30例。對照組1-3天予抗生素靜滴、1-7天予糖皮質(zhì)激素霧化吸入治療,治療組在此基礎(chǔ)上加用中藥煎劑清咽利喉方口服;治療組7-14天繼續(xù)予中藥煎劑清咽利喉方口服,對照組不予藥物治療。計(jì)算兩組患者術(shù)前1天、術(shù)后0天、術(shù)后7天、術(shù)后14天的臨床癥狀和體征積分,并對術(shù)后7天、術(shù)后14天進(jìn)行療效評價(jià)。研究結(jié)果:從治療前后臨床癥狀、體征總積分比較來看,兩組均有療效,治療組總體療效好于對照組;治療7天及14天后治療組在聲帶充血及次要癥狀上療效明顯優(yōu)于對照組;經(jīng)治療14天總體療效顯示:對照組治愈5例,顯效11例,有效6例,無效8例,治愈率為17%,總有效率73%;治療組治愈9例,顯效12例,有效7例,無效2例,治愈率為30%,總有效率為93%。3-6月后回訪:治療組無息肉復(fù)發(fā),對照組2例息肉復(fù)發(fā)。結(jié)論:清咽利喉方應(yīng)用于聲帶息肉術(shù)后治療對聲帶形態(tài)功能恢復(fù)具有一定療效,在術(shù)后早期對于減輕聲帶充血水腫比單純使用西藥治療效果更佳,同時(shí)明顯改善伴隨癥狀,可以加快聲帶修復(fù)、創(chuàng)面愈合。
[Abstract]:Objective: to observe the curative effect of Qingyan Lihou prescription, oral Chinese medicine decoction, after vocal cord polyp surgery. To evaluate the difference between Qingyan Lihou prescription and routine treatment, and to explore the effectiveness and excellence of Qingyan Lihou recipe in the recovery of vocal cord morphology and function after vocal cord polyps. Methods: from January 2016 to December 2016, the patients admitted to the department of otolaryngology of Jiangsu Provincial Hospital of traditional Chinese Medicine had surgical indications. According to the diagnostic criteria, 60 cases of "vocal cord polyp extraction under general anesthesia and larynx endoscope" were accepted after admission. The patients in the treatment group and the control group were divided into two groups: 30 cases in the treatment group and 30 cases in the control group. The control group was given antibiotic intravenous drip for 1-3 days. The treatment group was treated with glucocorticoid atomization inhalation for 1-7 days. Treatment group 7-14 days continue to take Chinese medicine decoction Qingyan Lihou oral, the control group no drug treatment. The two groups of patients 1 day before operation, 0 days after surgery, 7 days after surgery, 14 days after the clinical symptoms and signs score. The curative effect was evaluated on the 7th day and the 14th day after operation. Results: from the comparison of the total scores of clinical symptoms and signs before and after treatment, the two groups were all effective. The overall curative effect of the treatment group was better than that of the control group. After 7 days and 14 days of treatment, the curative effect of the treatment group was significantly better than that of the control group in terms of hyperemia and secondary symptoms of vocal cord. After 14 days of treatment, the results showed that 5 cases were cured in the control group, 11 cases were effective, 6 cases were effective, 8 cases were ineffective, the cure rate was 17%, the total effective rate was 73g. In the treatment group, 9 cases were cured, 12 cases were markedly effective, 7 cases were effective, 2 cases were ineffective, the cure rate was 30%, and the total effective rate was 93. 3. 6 months later: there was no recurrence of polyp in the treatment group. Conclusion: Qingyan Lihou prescription has a certain curative effect on the recovery of vocal cord morphology and function after the operation of vocal cord polyp. At the early stage of operation, the effect of reducing hyperemia and edema of vocal cord was better than that of using western medicine alone, and the accompanying symptoms were obviously improved, which could accelerate the repair of vocal cord and the healing of wound.
