【摘要】：目的:通過對(duì)過敏性紫癜(HSP)與紫癜性腎炎(HSPN)患兒瘀血積分、凝血相關(guān)指標(biāo)的觀察,探討?zhàn)鲅獙?duì)HSP患兒腎臟損傷的影響。方法:85例患兒按有無尿檢異常分為HSP組45例,HSPN組40例。根據(jù)中醫(yī)瘀血癥候積分表進(jìn)行瘀血評(píng)分。采用免疫比濁法測(cè)D-二聚體(D-Dimer),ELISA法測(cè)血漿與尿液纖溶酶原激活物抑制劑-1(PAI-1)。結(jié)果:與HSP組比較,HSPN組瘀血積分、尿PAI-1/Cr明顯增高(P0.05)。血D-Dimer、血PAI-1在HSP和HSPN組差異無統(tǒng)計(jì)學(xué)意義。尿PAI-1/Cr與瘀血積分、24h尿蛋白定量均呈正相關(guān)(P0.05)。結(jié)論:瘀血參與了HSP患兒腎臟損傷的發(fā)生,尿液中PAI-1水平的增高與HSP腎臟損傷有關(guān)。
[Abstract]:Objective: To explore the effect of blood stasis on renal injury in children with HSP, through observing the blood stasis and blood coagulation related indexes in children with Henoch Schonlein purpura (HSP) and purpura nephritis (HSPN). Methods: 85 cases according to whether the abnormal urine into HSP 45 cases, HSPN group of 40 cases. The blood stasis score was carried out according to the syndrome of blood stasis syndrome in Chinese medicine. D- two polymer (D-Dimer) was measured by immuno turbidimetry, and plasma and urine plasminogen activator inhibitor -1 (PAI-1) was measured by ELISA. Results: compared with the HSP group, the blood stasis score and the urinary PAI-1/Cr in the HSPN group were significantly higher (P0.05). There was no significant difference between the blood D-Dimer and the blood PAI-1 in the group of HSP and HSPN. The urine PAI-1/Cr was positively correlated with the blood stasis integral and the quantitative 24h urine protein (P0.05). Conclusion: blood stasis is involved in the occurrence of renal injury in children with HSP, and the increase of PAI-1 in urine is related to the renal injury of HSP.

【正文快照】： 過敏性紫癜(Henoch-Sch?nlein purpura,HSP)是以小血管炎病變?yōu)榛A(chǔ)的全身性血管炎綜合征。臨床表現(xiàn)主要為非血小板減少性可觸性皮膚紫癜,常伴腹痛、關(guān)節(jié)痛,20%-60%的HSP患兒易發(fā)生腎臟損傷,出現(xiàn)血尿、蛋白尿等癥狀,即紫癜性腎炎(Henoch-Sch?nlein purpura nephritis,HSPN)[1]
