【摘要】：目的:本研究旨在通過(guò)觀察參芪六味湯對(duì)脾虛痰濕型冠心病穩(wěn)定型心絞痛的中醫(yī)癥候、心電圖、硝酸甘油用量、心絞痛癥狀及C-反應(yīng)蛋白的變化和影響,進(jìn)而探究其臨床療效。方法:在2015年2月至2015年12月長(zhǎng)春中醫(yī)藥大學(xué)附屬醫(yī)院心病二科住院部及門(mén)診,選擇符合納入標(biāo)準(zhǔn)的穩(wěn)定型心絞痛患者60例,中醫(yī)辨證為脾虛痰濕證,用簡(jiǎn)單隨機(jī)法中的擲骰子法分為治療組與對(duì)照組。比例為1:1,即每組30例。治療組給予基礎(chǔ)西藥加用參芪六味湯治療,其中基礎(chǔ)西藥治療包括(可根據(jù)臨床實(shí)際情況調(diào)整用藥):阿司匹林腸溶片100mg/日,酒石酸美托洛爾片100mg/日,阿托伐他汀鈣片20mg/日;對(duì)照組給予基礎(chǔ)西藥治療。臨床觀察共為期4周。觀察治療組和用藥組病例用藥前后的臨床癥狀,記錄中醫(yī)癥候評(píng)分、硝酸甘油的服用量、心電圖、心絞痛癥狀以及C-反應(yīng)蛋白,記錄并分析前后數(shù)據(jù)的變化。結(jié)果:1、參芪六味湯能明顯降低脾虛痰濕型冠心病穩(wěn)定型心絞痛患者的中醫(yī)癥候積分,對(duì)心絞痛的癥狀有明顯的改善;2、通過(guò)對(duì)比發(fā)現(xiàn),參芪六味湯與基礎(chǔ)西藥聯(lián)合使用可以改善冠心病穩(wěn)定型心絞痛患者的心電圖;3、通過(guò)臨床觀察,參芪六味湯與基礎(chǔ)西藥聯(lián)合使用可以有效減少冠心病穩(wěn)定型心絞痛患者硝酸甘油用量;4、通過(guò)對(duì)比觀察,參芪六味湯可改善冠心病穩(wěn)定型心絞痛患者的心絞痛癥狀。5、通過(guò)臨床試驗(yàn)觀察,未發(fā)現(xiàn)患者服用參芪六味湯后有不良反應(yīng)發(fā)生。結(jié)論:參芪六味湯能改善冠心病穩(wěn)定型心絞痛患者的臨床癥狀,改善心電圖心肌缺血情況,減少硝酸甘油的服用量,療效安全、確切,值得臨床推廣,未來(lái)有廣泛的前景。
[Abstract]:Objective: To observe the clinical efficacy of Shenqi six flavor Decoction on TCM syndrome, electrocardiogram, nitroglycerin consumption, angina pectoris symptoms and C- reactive protein in patients with stable angina pectoris due to spleen deficiency and phlegm dampness type. Methods: from February 2015 to December 2015 in the heart of Affiliated Hospital of Changchun University of Traditional Chinese Medicine two Department of inpatient and outpatient, with a total of 60 patients with stable angina pectoris standard, TCM syndrome of the spleen with phlegm, using dice method simple random method were divided into treatment group and control group. The proportion was 1:1, that is, 30 cases in each group. The treatment group was given Shenqi decoction plus Western medicine treatment six, which is the basis of Western medicine treatment (including medication can be adjusted according to the clinical conditions): Aspirin Enteric-coated Tablets 100mg/, Metoprolol Tartrate Tablets 100mg/, Atorvastatin Calcium Tablets 20mg/; the control group was given the basic treatment of Western medicine. The clinical observation lasted for 4 weeks. Observe the clinical symptoms before and after treatment in the treatment group and the medication group, record the TCM symptom score, the dosage of nitroglycerin, the electrocardiogram, angina pectoris symptoms and C- reactive protein, record and analyze the changes of the data before and after treatment. Results: 1, six Shenqi decoction can significantly reduce the TCM symptoms of spleen deficiency type patients with stable angina pectoris of coronary heart disease, the angina symptoms were obviously improved; 2, by comparison, combined with Shenqi decoction and Western medicine six can improve electrocardiogram in patients with stable angina pectoris of coronary heart disease; 3, through clinical observation, combined with Shenqi decoction and Western medicine six can effectively reduce the stable angina pectoris of coronary heart disease patients with the dosage of nitroglycerin; 4, by comparison, six Shenqi decoction can improve the patients with stable angina pectoris of coronary heart disease angina pectoris. 5, through clinical trial observation, no patients were found to take Shenqi six flavour soup after the adverse reaction. Conclusion: Shenqi six flavor decoction can improve the clinical symptoms of coronary heart disease patients with stable angina pectoris, improve the myocardial ischemia and reduce the dosage of nitroglycerin, which is safe and accurate. It is worthy of clinical promotion and has wide prospects in the future.
