【摘要】：目的:研究活血化瘀通絡(luò)方治療早期激素性股骨頭壞死的臨床價(jià)值。方法:選取2015年3月至2017年4月我院收治的早期激素性股骨頭壞死患者90例為研究對(duì)象,采用隨機(jī)數(shù)表法分為觀察組、對(duì)照組各45例,均于內(nèi)固定術(shù)后2周內(nèi)進(jìn)行旋股內(nèi)、外側(cè)動(dòng)脈介入術(shù),術(shù)后對(duì)照組行常規(guī)治療,觀察組加服活血化瘀通絡(luò)方,比較兩組治療有效率及治療前后血清骨形成蛋白2(BMP2)、骨鈣素(BGP)、轉(zhuǎn)化生長(zhǎng)因子-β1(TGF-β1)、內(nèi)皮素(ET)水平,分析兩組血漿黏度(ηp)、紅細(xì)胞壓積(HCT)、紅細(xì)胞沉降率(ESR)等血液流變學(xué)指標(biāo),采用視覺(jué)模擬評(píng)分法(VAS)、Harris髖關(guān)節(jié)功能評(píng)分表評(píng)價(jià)兩組疼痛度、關(guān)節(jié)功能,同時(shí)記錄不良反應(yīng)。結(jié)果:觀察組治療有效率88.89%,明顯高于對(duì)照組71.11%;觀察組治療后血清BMP2(23.10±1.54)μg/L、BGP(38.20±1.11)pg/ml、TGF-β1(31.29±1.35)pg/ml均明顯高于對(duì)照組,而ET水平(50.28±1.10)pg/ml明顯低于對(duì)照組;治療后觀察組血ηp(1.02±0.21)mP a·s、HCT(25.10±1.24)%、ESR(12.19±0.14)mm/h明顯低于對(duì)照組;治療后觀察組VAS評(píng)分(1.23±0.14)分明顯低于對(duì)照組,Harris評(píng)分(89.45±1.13)分明顯高于對(duì)照組;兩組不良反應(yīng)發(fā)生率11.11%、8.89%比較無(wú)顯著差異。結(jié)論:活血化瘀通絡(luò)方可有效治療早期激素性股骨頭壞死,改善其骨代謝及血液流變學(xué)指標(biāo),降低疼痛度,促進(jìn)機(jī)體關(guān)節(jié)功能恢復(fù),同時(shí)安全性好,值得在臨床推廣應(yīng)用。
[Abstract]:Objective: To study the clinical value of Huoxue Huayu Tongluo Recipe (Huoxue Huayu Tongluo recipe) in the treatment of early hormone necrosis of the femoral head. Methods: from March 2015 to April 2017 in our hospital, the avascular necrosis of the femoral head in 90 patients as the research object, divided into observation group and control group with 45 cases in each group randomly, both in internal fixation within 2 weeks after the lateral and medial femoral circumflex artery intervention after surgery, the control group received routine treatment, the observation group with oral Huoxue Huayu Tongluo recipe, compared two groups of the effective rate of the treatment and treatment of serum bone morphogenetic protein 2 (BMP2) and Osteocalcin (BGP), transforming growth factor beta 1 (TGF- beta 1) and endothelin (ET) level, the analysis of plasma viscosity of two groups (N P), red blood cell hematocrit (HCT), erythrocyte sedimentation rate (ESR) and blood rheology indicators, using the visual analogue scale (VAS), Harris hip score evaluation of two groups of pain, joint function, and record adverse reactions. Results: in the observation group, the total effective rate was 88.89%, 71.11% higher than the control group; the observation group after treatment serum BMP2 (23.10 + 1.54) g/L, BGP (38.20 + 1.11) pg/ml, TGF- beta 1 (31.29 + 1.35) pg/ml were significantly higher than the control group, while the level of ET (50.28 + 1.10) pg/ml was significantly lower than the control group after treatment; the observation group of blood p (1.02 + 0.21) mP a - s, HCT (25.10 + 1.24)%, ESR (12.19 + 0.14) mm/h was significantly lower than the control group; after treatment, the VAS score of the observation group (1.23 + 0.14) was significantly lower than the control group, Harris score (89.45 + 1.13) points significantly higher than the control group; the adverse effects of the two groups, there is no significant difference between 8.89% and 11.11%. Conclusion: Huoxue Huayu Tongluo Decoction can effectively treat steroid induced avascular necrosis of the femoral head, improve its bone metabolism and hemorheology index, reduce pain and promote joint function recovery. Meanwhile, it has good safety and is worthy of clinical application.
