【摘要】：中醫(yī)歷經數千年的傳承與發(fā)展,為人類健康長壽做出了諸多杰出的貢獻。中醫(yī)的發(fā)展史是一部人類與疾病抗爭的奮斗史,其發(fā)展有賴于歷代醫(yī)家不斷的傳承與創(chuàng)新。上至四大經典的問世,下至如今眾多的醫(yī)學典籍的涌現,每一部著作都承載著無數醫(yī)者的心血,這些理論用于指導臨床的同時,再經由臨床的不斷驗證及創(chuàng)新,新的理論及方法又不斷地更新及豐富了現有的理論。因此,對于醫(yī)家學術著作的保存及整理發(fā)掘,有利于豐富中醫(yī)理論及推動臨床實踐的發(fā)展。本課題正是基于上述背景下,通過對當代“內科雜病專家”彭憲彰老師現存著作及與相關的文獻資料進行整理和深入研究,歸納與分析彭憲彰老師的學術思想及臨床辨治經驗,期望能將彭老的學術思想及臨床經驗進行系統(tǒng)地整理,能夠全面集中地呈現彭老的畢生成就,為中醫(yī)理論及臨床辨治添磚加瓦。本論文采用文獻研究的方法,將彭憲彰老師現存的著作及病案進行整理分類,分析并總結彭老的學術思及臨床辨治經驗的特點,此外文中選取彭老具有典型特征的病案進行舉證,以便于本文進行分析與闡述。本論文從彭憲彰所撰寫的《傷寒六十九論》及《葉氏醫(yī)案存真疏注》兩部著作中可以看出,彭老對傷寒學術及溫病學術均有很深的造詣,尤其長于臨床應用。彭老擅長使用經方治療雜證,并首創(chuàng)麻杏石甘湯治療小兒遺尿癥,開創(chuàng)以宣法治療遺尿癥的先河,有助于開拓中醫(yī)辨證思路。結合溫熱學派的辨治體系,根據病情現狀自擬處方治療溫熱諸證。臨床用藥具有“簡、效、聯、變、和”的五大特點。處方用藥重視顧護“后天之本”兼顧脾胃,不喜用攻伐之藥,在治療中總以用藥平和為特點,考慮患者體質的同時,兼顧四時地域特征,具有“天人相應”的用藥特點。此外,彭老治學態(tài)度嚴謹,辨治思維靈活變通,在批判性地繼承前人學術思想及臨床經驗的基礎上,勇于不斷地創(chuàng)新。
[Abstract]:After thousands of years of inheritance and development, traditional Chinese medicine has made many outstanding contributions to the health and longevity of human beings. The history of the development of traditional Chinese medicine is a struggle history of human and disease, and its development depends on the continuous inheritance and innovation of the doctors of the past dynasties. From the four to the advent of the classic, now many medical books emerged, every book was carrying countless medical efforts, these theories to guide clinic at the same time, through continuous validation and clinical innovation, new theories and methods and constantly update and enrich the existing theory. Therefore, the preservation and collation of medical academic works is conducive to enriching the theory of traditional Chinese medicine and promoting the development of clinical practice. This paper is based on the above background, the contemporary "miscellaneous diseases expert Peng Xianzhang teacher existing works and relevant literature and research, induction and analysis of Professor Peng Xianzhang's academic thought and clinical experience, is expected to be Peng Lao's academic thoughts and clinical experience systematically and comprehensively to show Peng Lao's accomplishments, to contribute to the theory of traditional Chinese medicine and clinical treatment. This paper uses the methods of literature research, Peng Xianzhang teacher extant writings and medical records were collected and classified, analyzed and summarized the characteristics of the old Peng's clinical experience and academic thinking, and the selection of Peng Lao has the typical characteristics of the medical evidence, to facilitate the analysis and elaboration. This thesis is written from Peng Xianzhang's "typhoid theory" and "sixty-nine note" true records of Ye Shu two works can be seen in the academic and academic Peng Lao on typhoid has deep knowledge of epidemic febrile diseases, especially at clinical application. Peng Lao is good at using classical prescriptions to treat miscellaneous syndrome, and first Maxingshigan Decoction in the treatment of infantile enuresis, to create the solution treatment of enuresis, helps develop TCM thinking. Combined with the system's warm school, according to the disease status of self-made prescription in treating febrile syndromes. Clinical medication has five characteristics: "simple, effective, combined, variable, and". Prescription medication pays much attention to the "nurture of the acquired nature", which is both the spleen and stomach, and does not like to use the attack cutting medicine. In the treatment, it is characterized by the level of medication. Considering the constitution of the patients, it also takes into account the regional characteristics of four seasons, and has the characteristics of "heaven and man correspondence". In addition, Peng Laozhi learning attitude is rigorous, dialectical thinking flexibly, based on inherits the academic thought and clinical experience, the courage to innovate.
