第一部分青年結(jié)腸癌的臨床病理特征研究背景和目的:結(jié)直腸癌多發(fā)于老年人,90%以上發(fā)生于55歲以上。近年來隨著在老年人群中普及腫瘤篩查和纖維腸鏡檢查,結(jié)直腸癌的總發(fā)病率和死亡率明顯下降,而青年結(jié)直腸癌（early-onset colorectal cancer,EOCRC）的發(fā)病率卻在逐年上升。青年結(jié)直腸癌是區(qū)別于老年結(jié)直腸癌的獨(dú)立亞型,具有獨(dú)特的臨床病理特征和預(yù)后。青年結(jié)直腸癌常被定義為一種侵襲性疾病,診斷時(shí)分期晚,組織學(xué)分化差,粘液腺癌、印戒細(xì)胞癌常見,預(yù)后差。左右半結(jié)腸癌從胚胎起源到流行病學(xué)、臨床病理特征、分子特征、治療及預(yù)后方面均有明顯差異,其相關(guān)研究愈來愈受到重視。目前國(guó)內(nèi)外在青年結(jié)直腸癌中左右半結(jié)腸的相關(guān)研究很少,亟待大樣本的數(shù)據(jù)分析以更好的指導(dǎo)臨床工作。本研究旨在探索≤40歲青年結(jié)腸癌的臨床病理特征及左、右半結(jié)腸癌的差異,從而為今后青年結(jié)腸癌的治療和預(yù)后提供新的思路。方法:收集2004年1月至2014年12月期間山東大學(xué)齊魯醫(yī)院手術(shù)切除并經(jīng)病理確診的年齡≤4 0歲結(jié)腸癌患者18 7例,其中4例家族性腺瘤性息肉�。╢amilial adenomatous polyposis,...
