二維超聲征象預測PTC風險的分子病理學研究

【摘要】:目的:通過結合二維超聲圖像特征與長鏈非編碼RNA HOTAIR在甲狀腺乳頭狀癌中的分子病理學研究,探討二維超聲圖像特征術前評估甲狀腺乳頭狀癌風險的價值。方法:1.從TCGA數(shù)據(jù)庫下載HOTAIR基因表達譜及臨床參數(shù)信息,分析HOTAIR在甲狀腺乳頭狀癌中的表達規(guī)律及其與臨床病理參數(shù)的相關性。獨立樣本t檢驗比較兩組樣本中HOTAIR的表達水平;卡方檢驗檢測HOTAIR表達水平與臨床病理參數(shù)的相關性;Kaplan-Meier曲線對比HOTAIR高表達組與HOTAIR低表達組患者的生存時間。2.回顧性歸納總結34例甲狀腺乳頭狀癌病灶的二維超聲圖像特征,包括數(shù)目、大小、邊緣、縱橫比、內部回聲、鈣化。3.應用RT-q PCR檢測34例甲狀腺乳頭狀癌及10例甲狀腺正常組織中HOTAIR的表達水平,獨立樣本t檢驗比較兩組樣本中HOTAIR的表達水平。4.卡方檢驗或Fisher確切概率法分析各二維超聲圖像特征與HOTAIR表達規(guī)律的相關性。結果:TCGA數(shù)據(jù)庫統(tǒng)計結果表明甲狀腺乳頭狀癌組織中HOTAIR表達水平較甲狀腺正常組織明顯升高(P0.05),HOTAIR高表達與腫瘤大小(P=0.032)、淋巴結轉移(P=0.009)、TNM分期(P=0.038)相關;HOTAIR高表達的甲狀腺乳頭狀癌患者總生存時間明顯短于低表達的患者(P=0.03);所納入病例病灶的二維超聲圖像多表現(xiàn)為邊緣不規(guī)則(91.2%)、縱橫比≥1(76.5%)、實性低回聲(94.1%)、伴微鈣化(67.6%);RT-q PCR方法檢測的納入病例樣本中HOTAIR表達水平與TCGA數(shù)據(jù)庫統(tǒng)計結果一致,同樣表現(xiàn)為在甲狀腺乳頭狀癌組織中表達上調(P0.05),其中縱橫比≥1與HOTAIR高表達有相關。結論:HOTAIR可能參與了甲狀腺乳頭狀癌的發(fā)生發(fā)展過程,是甲狀腺乳頭狀癌的不良預后因素;甲狀腺乳頭狀癌圖像特征中縱橫比≥1與長鏈非編碼RNA HOTAIR的高表達相關,是甲狀腺乳頭狀癌術前風險評估的危險因素,可為其個體化治療提供依據(jù)。
[Abstract]:Aim: to evaluate the risk of thyroid papillary carcinoma by combining the characteristics of two-dimensional ultrasound and long-chain non-coding RNA HOTAIR in the study of molecular pathology of papillary thyroid carcinoma. Methods: 1. HOTAIR gene expression profile and clinical parameter information were downloaded from TCGA database to analyze the expression pattern of HOTAIR in papillary thyroid carcinoma and its correlation with clinical pathological parameters. Independent sample t test was used to compare the expression level of HOTAIR in two groups; Chi square test was used to detect the correlation between HOTAIR expression level and clinicopathological parameters; Kaplan-Meier curve was used to compare the survival time of patients with high expression of HOTAIR and low expression of HOTAIR. The ultrasonographic features of 34 cases of papillary thyroid carcinoma were retrospectively summarized, including number, size, margin, aspect ratio, internal echo, calcification. RT-q PCR was used to detect the expression of HOTAIR in 34 cases of papillary thyroid carcinoma and 10 cases of normal thyroid tissue. T test of independent sample was used to compare the expression level of HOTAIR in the two groups. Chi-square test or Fisher exact probability method was used to analyze the correlation between the characteristics of each two-dimensional ultrasound image and the expression of HOTAIR. Results: the results of TCGA database showed that the expression level of HOTAIR in papillary thyroid carcinoma was significantly higher than that in normal thyroid tissues (P0.05), the expression of), HOTAIR was higher than that in normal thyroid tissues (P0. 032), lymph node metastasis (P < 0. 009), and tumor size (P < 0. 032).
