【摘要】：目的比較腹腔鏡輔助胃癌根治術(shù)與開(kāi)腹胃癌根治術(shù)的短期臨床效果。方法選取2014年1—12月于本院普通外科住院并診斷為胃癌的患者185例,男105例,女80例,年齡31~85(55±6)歲,其中腹腔鏡輔助胃癌根治術(shù)86例(腹腔鏡組),開(kāi)腹胃癌根治術(shù)99例(開(kāi)腹組),兩組資料具有可比性(P0.05);比較兩組短期臨床效果。結(jié)果與開(kāi)腹組相比,腔鏡組手術(shù)切口直徑短[(5.8±0.5)cm vs(14.1±1.3)cm,P0.05],手術(shù)時(shí)間長(zhǎng)[(190.9±50.3)min vs(150.7±43.1)min,P0.05],術(shù)中出血量少[(107.3±70.3)ml vs(158.3±90.8)ml,P0.05],術(shù)后排氣時(shí)間短[(3.0±0.4)d vs(3.9±0.5)d,P0.05],進(jìn)食時(shí)間早[(3.7±0.9)d vs(4.3±0.7)d,P0.05],住院時(shí)間短[(9.6±3.4)d vs(12.2±6.9)d,P0.05],差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組的手術(shù)切除淋巴結(jié)數(shù)目[(26.4±7.5)vs(27.2±7.7),P0.05]、淋巴結(jié)陽(yáng)性數(shù)[(3.0±1.0)vs(3.0±0.9),P0.05)]、切緣陽(yáng)性率(1.16%vs 1.01%)和術(shù)后并發(fā)癥發(fā)生率(4.6%vs 5.0%)差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論傳統(tǒng)開(kāi)腹胃癌根治手術(shù)能夠徹底切除病灶,但切口大、手術(shù)出血量大,術(shù)后恢復(fù)慢。而腹腔鏡輔助胃癌根治術(shù)手術(shù)創(chuàng)傷小,術(shù)后患者恢復(fù)快,可獲得較好短期臨床效果,優(yōu)勢(shì)明顯,但手術(shù)時(shí)間稍長(zhǎng)。
[Abstract]:Objective to compare the short-term clinical effects of laparoscopic assisted radical gastrectomy and open radical gastrectomy. Methods from January to December 2014, 185 patients (105 males and 80 females, aged 31 ~ 85 (55 鹵6) years) who were hospitalized in general surgery and diagnosed as gastric cancer in our hospital from January to December 2014 were selected. Among them, 86 cases were treated with laparoscopic assisted radical gastrectomy (Laparoscopic group). There were 99 cases of radical gastrectomy (open group). The data of the two groups were comparable (P0.05). To compare the short-term clinical effect between the two groups. Results compared with the open group, the operative incision diameter was shorter in the endoscopic group [(5.8 鹵0.5) cm vs (14.1 鹵1.3) cm,P0.05], and the operative time was longer [(190.9 鹵50.3) min vs () 150.7 鹵43.1 min,P0.05]. The blood loss was less during operation [(107.3 鹵70.3) ml vs () 158.3 鹵90.8 ml,P0.05], and the postoperative exhaust time was shorter [(3.0 鹵0.4) d vs (3.9 鹵0.5) d (P0.05)]. The feeding time was early [(3.7 鹵0.9) d vs (4.3 鹵0.7) dU P0.05], the hospitalization time was short [(9.6 鹵3.4) d vs (12.2 鹵6.9) dP05], the difference was statistically significant (P0.05). The number of lymph nodes resected in both groups [(26.4 鹵7.5) vs (27.2 鹵7.7), P0.05], the number of positive lymph nodes [(3.0 鹵1.0) vs (3.0 鹵0.9), P0.05], The positive rate of incision margin (1.16%vs 1.01%) and the incidence of postoperative complications (4.6%vs 5.0%) had no significant difference (P0.05). Conclusion the traditional radical operation for gastric cancer can completely remove the lesions, but the incision is large, the amount of bleeding is large, and the recovery is slow. Laparoscopy-assisted radical gastrectomy has less trauma, faster recovery, and better short-term clinical effect, but the operation time is longer than that of laparoscopically assisted radical gastrectomy.
