【摘要】：[目的]探討分析腹腔鏡胃癌根治術(shù)與傳統(tǒng)開腹胃癌根治術(shù)治療胃癌的臨床療效,闡述腹腔鏡技術(shù)在臨床的應(yīng)用價(jià)值和治療疾病的可行性。[方法]回顧性分析2013年至今我科治療的56例胃癌患者,其中30例行腹腔鏡胃癌根治術(shù),26例行開腹胃癌根治術(shù)。患者的選擇上,腫瘤生長部位都在胃竇部,且病理學(xué)證實(shí)為胃腺癌。根據(jù)腹腔鏡和開腹分成兩組,對(duì)照組(op組)和實(shí)驗(yàn)組(Lap組)。兩組患者的年齡,性別及其它基礎(chǔ)病無明顯差異。對(duì)兩組患者進(jìn)行手術(shù)后分析,對(duì)比其手術(shù)時(shí)間、術(shù)中出血量、切口長度、淋巴結(jié)清掃情況、術(shù)后并發(fā)癥等來評(píng)價(jià)腹腔鏡胃癌根治術(shù)和開腹胃癌根治術(shù)的優(yōu)缺點(diǎn),證明腹腔鏡胃癌根治術(shù)在臨床應(yīng)用的前景。[結(jié)果]兩組患者都順利的完成了根治性手術(shù)的治療。我們分別對(duì)這兩組患者進(jìn)行了術(shù)中后術(shù)后情況的觀察和對(duì)比分析,在手術(shù)時(shí)間上(P0.05)有顯著差異,開腹組要快于腹腔鏡組。在切口長度、術(shù)中出血、術(shù)后恢復(fù)及并發(fā)癥的發(fā)生上(P0.05)有統(tǒng)計(jì)學(xué)差異,腹腔鏡組要優(yōu)于開腹組。在淋巴結(jié)清除情況、腫瘤切除情況,術(shù)后遠(yuǎn)期復(fù)發(fā)情況上(P0.05)無統(tǒng)計(jì)學(xué)差異,兩組手術(shù)情況無明顯差異。[結(jié)論]腹腔鏡胃癌根治術(shù)與開腹手術(shù)相比,在手術(shù)切口長度,術(shù)中出血、術(shù)后恢復(fù)及并發(fā)癥的控制上要優(yōu)于開腹手術(shù)。兩種手術(shù)在遠(yuǎn)期療效上無明顯差異,腹腔鏡胃癌根治術(shù)可以作為一種新的術(shù)式推廣研究。隨著腹腔鏡技術(shù)的發(fā)展及器械的進(jìn)步和大量的實(shí)驗(yàn)研究,其在臨床上的應(yīng)用價(jià)值會(huì)進(jìn)一步擴(kuò)大提高,對(duì)醫(yī)學(xué)及患者有著深遠(yuǎn)的影響。
[Abstract]:[objective] to explore the clinical effect of laparoscopic radical gastrectomy and traditional open radical gastrectomy for gastric cancer, and to expound the clinical application value and the feasibility of laparoscopic technique in the treatment of gastric cancer. [methods] 56 cases of gastric cancer treated in our department from 2013 to now were retrospectively analyzed. 30 cases underwent laparoscopic radical gastrectomy and 26 cases underwent open radical gastrectomy. The tumor was located in the antrum and pathologically proved to be gastric adenocarcinoma. According to laparoscopy and laparotomy were divided into two groups: control group (op group) and experimental group (Lap group). There was no significant difference in age, sex and other underlying diseases between the two groups. To evaluate the advantages and disadvantages of laparoscopic radical gastrectomy and open radical gastrectomy, we compared the operation time, intraoperative bleeding volume, incision length, lymph node dissection and postoperative complications between the two groups. To prove the prospect of laparoscopic radical gastrectomy in clinical application. [results] the two groups of patients successfully completed the treatment of radical surgery. We observed and compared the operation time of the two groups after operation (P0.05), the open group was faster than the laparoscopic group. There were significant differences in incision length, intraoperative bleeding, postoperative recovery and complications (P0.05). The laparoscopy group was better than the open group. There was no significant difference in lymph node clearance, tumor resection and long-term recurrence (P0.05), but there was no significant difference between the two groups. [conclusion] Laparoscopic radical gastrectomy is superior to open surgery in length of incision, bleeding, postoperative recovery and control of complications. There is no significant difference between the two operations in long-term effect. Laparoscopic radical gastrectomy can be used as a new operative method to popularize the study. With the development of laparoscopic technology, the progress of instruments and a large number of experimental studies, its clinical application value will be further expanded and improved, which has a profound impact on medicine and patients.
