胃泌素釋放肽前體聯(lián)合神經(jīng)元特異性烯醇化酶對(duì)小細(xì)胞肺癌化療療效的評(píng)價(jià)

【摘要】:目的:化療是治療肺癌的關(guān)鍵手段之一,對(duì)化療療效的評(píng)價(jià)非常重要,Pro GRP和NSE在SCLC中都有不同程度表達(dá),本實(shí)驗(yàn)旨在研究SCLC患者化療前后血清Pro GRP及NSE水平變化,探討Pro GRP和NSE是否有望成為預(yù)測(cè)SCLC化療療效的可靠指標(biāo)。方法:健康對(duì)照組來(lái)自弋磯山醫(yī)院體檢中心,SCLC患者經(jīng)病理學(xué)組織確診,存在可評(píng)價(jià)病灶,符合化療適應(yīng)證。SCLC患者化療前1天及化療2、4周期后20天和健康對(duì)照組抽取靜脈血液3 m L,離心取上層血清于零下80℃下凍存。使用ELISA方法測(cè)定血清Pro GRP和NSE水平在化療前后的變化。結(jié)果:一、(1)與對(duì)照組比較,SCLC患者化療前與化療2周期后Pro GRP、NSE濃度明顯高于正常對(duì)照組(P≤0.05);化療4周期后與對(duì)照組比較,P0.05。(2)化療前后對(duì)比,化療前分別與化療2、4周期后Pro GRP,NSE濃度比較,P≤0.05;化療2、4周期后相比,P0.05。二、(1)各組中化療前后Pro GRP比較,CR(完全緩解)、SD(穩(wěn)定)、PD(進(jìn)展)組顯示化療前和化療2、4周期后兩兩比較,P≤0.05;PR(部分緩解)組,化療前分別與化療2、4周期后相比,P≤0.05;化療2、4周期后相比,P0.05。(2)各組中化療前后NSE比較,CR、PR、SD組顯示化療前分別與2、4周期后比較,P≤0.05;化療2、4周期后相比,P0.05。PD組化療前和化療2周期后比較,化療2、4周期后相比,P≤0.05;化療前和化療4周期后相比,P0.05。三、(1)化療前4組比較,CR、PR、SD、PD組Pro GRP、NSE濃度比較,P0.05。(2)化療2周期后4組比較,CR組與PR、SD、PD組Pro GRP比較,P≤0.05;PR、SD、PD組Pro GRP比較,P0.05。CR與SD、PD組NSE比較,PR與PD組NSE比較,P≤0.05;PR與CR、SD組NSE比較,SD、PD組NSE比較,P0.05。(3)化療4周期后,CR、PR、SD組Pro GRP比較,P0.05;PD組與CR、PR、SD組比較,P≤0.05。NSE比較與化療2周期后NSE化療前后比較相似,除SD、PD組比較,P≤0.05。結(jié)論:1.化療前Pro GRP、NSE濃度明顯高于健康人群,化療后Pro GRP、NSE的濃度均有不同程度下降,以化療2周期后下降明顯。2.SCLC患者化療2、4周期后Pro GRP、NSE濃度比較,P0.05,但Pro GRP、NSE水平仍呈小幅下降趨勢(shì)。3.化療后CR、PR、SD、PD組血清Pro GRP、NSE濃度下降幅度不一,CR組下降明顯,Pro GRP對(duì)化療療效的監(jiān)測(cè)更有意義。化療前后血清Pro GRP、NSE濃度之間呈正相關(guān)。4.Pro GRP、NSE聯(lián)合檢測(cè)SCLC與單獨(dú)檢測(cè)的敏感性差異不大,特異性較單獨(dú)檢測(cè)高。
