【摘要】：目的:探討多發(fā)性骨髓瘤繼發(fā)骨髓纖維化的骨髓活檢病理特征及其與疾病預(yù)后的關(guān)系。方法:回顧性分析2012年1月1日至12月31日期間本院血液內(nèi)科收治的70例(其中35例患者繼發(fā)骨髓纖維化)多發(fā)性骨髓瘤患者臨床資料。觀察患者骨髓活檢病理學(xué)特征,對(duì)比分析骨髓涂片及活檢增生程度、治療后纖維化程度的變化,同時(shí)比較繼發(fā)骨髓纖維化與未發(fā)生骨髓纖維化患者之間總死亡率的差異。結(jié)果:各期、各分型多發(fā)性骨髓瘤均可繼發(fā)骨髓纖維化,骨髓纖維化可致骨髓涂片出現(xiàn)假性增生減低,繼發(fā)骨髓纖維化患者骨髓纖維組織不同程度增生,Gomori染色陽(yáng)性(+)至強(qiáng)陽(yáng)性(+++),部分患者疾病好轉(zhuǎn)后骨髓纖維化程度可減輕。結(jié)論:多發(fā)性骨髓瘤繼發(fā)骨髓纖維化時(shí)骨髓活檢對(duì)診斷有重要價(jià)值,繼發(fā)骨髓纖維化患者較未發(fā)生纖維化患者總體預(yù)后欠佳。
[Abstract]:Objective: to investigate the pathological features of bone marrow biopsy and its relationship with the prognosis of multiple myeloma secondary to bone marrow fibrosis. Methods: the clinical data of 70 patients with multiple myeloma from January 1 to December 31, 2012 were retrospectively analyzed. The pathological features of bone marrow biopsy were observed and the degree of bone marrow smear and biopsy hyperplasia were compared and the changes of fibrosis degree after treatment were analyzed. The difference of total mortality between the patients with secondary bone marrow fibrosis and those without bone marrow fibrosis was also compared. Results: all types of multiple myeloma could be secondary to myelofibrosis, and myelofibrosis could lead to pseudoproliferative decrease in bone marrow smears and proliferation of bone marrow fiber tissue in patients with secondary myelofibrosis. The degree of bone marrow fibrosis can be alleviated after the improvement of disease in some patients with Gomori positive () to strong positive (),. Conclusion: bone marrow biopsy in patients with multiple myeloma secondary to bone marrow fibrosis is of great value in the diagnosis, and the prognosis of the patients with secondary myeloma fibrosis is worse than that of the patients without fibrosis.
