維生素D受體在胃癌組織中的表達(dá)及臨床意義

【摘要】:背景及目的近年來(lái)大量的細(xì)胞、動(dòng)物學(xué)實(shí)驗(yàn)研究結(jié)果表明維生素D具有廣泛的抗腫瘤作用,然而流行病學(xué)調(diào)查結(jié)果并不支持維生素D是胃癌的保護(hù)性因素這一說(shuō)法,目前認(rèn)為其生物學(xué)功能包括抗腫瘤作用須由維生素D受體(VDR)介導(dǎo),于此我們大膽猜測(cè)胃癌組織中VDR可能異常表達(dá)(低表達(dá)),本研究旨在檢測(cè)胃癌組織、癌旁正常胃粘膜組織以及癌前病變胃粘膜組織中VDR的表達(dá)情況并分析其臨床意義。材料及方法采用免疫組織化學(xué)法檢測(cè)不同類型胃粘膜組織中VDR的表達(dá)情況,卡方檢驗(yàn)法檢驗(yàn)其表達(dá)差異有無(wú)統(tǒng)計(jì)學(xué)意義,并分析其與胃癌患者臨床各項(xiàng)參數(shù)間的關(guān)系。胃癌組織及癌旁正常胃粘膜組織來(lái)源于甘肅省人民醫(yī)院92例行胃癌根治術(shù)患者的手術(shù)標(biāo)本,癌前病變組織包括慢性萎縮性胃炎(50例)、腸上皮化生(52例)、不典型增生(46例)均收集于甘肅省人民醫(yī)院內(nèi)鏡中心。結(jié)果胃粘膜組織中VDR主要表達(dá)于細(xì)胞質(zhì),胞核基本無(wú)表達(dá)。相較于癌旁正常胃粘膜(82.61%)及癌前病變胃粘膜組織(73.64%),VDR在胃癌組織中呈低表達(dá)狀態(tài)(57.61%),具有明顯的統(tǒng)計(jì)學(xué)差異(P=0.001)。在胃癌組織中,高、中、低分化組間VDR表達(dá)量依次降低,差異具有統(tǒng)計(jì)學(xué)意義(P=0.016);相較于小瘤體組(5 cm),其在大瘤體組(≥5 cm)中低表達(dá),同樣具有統(tǒng)計(jì)學(xué)差異(P=0.009)。比較不同類型胃粘膜組織中VDR陽(yáng)性表達(dá)率,結(jié)果顯示隨著胃癌的發(fā)生發(fā)展,其表達(dá)呈一下降趨勢(shì)。結(jié)論胃粘膜組織細(xì)胞中VDR主要定位于胞質(zhì),提示維生素D類藥物對(duì)胃癌細(xì)胞的作用可能限于非基因途徑或其基因途徑較復(fù)雜;VDR在胃癌組織尤其是低分化胃癌組織中呈低表達(dá)狀態(tài);瘤體越大的胃癌組織VDR含量越低;VDR低表達(dá)并不僅僅發(fā)生在胃癌形成后,其異常表達(dá)始于癌前病變。VDR可能會(huì)成為判斷胃癌預(yù)后的一項(xiàng)指標(biāo)。
[Abstract]:Background and objective in recent years, a large number of cells, animal experimental results show that vitamin D has a wide range of anti-tumor effects, but epidemiological findings do not support the claim that vitamin D is a protective factor for gastric cancer. It is believed that its biological function including antitumor action should be mediated by vitamin D receptor (VDR). In this study, we venture to speculate that there may be abnormal expression (low expression) of VDR in gastric cancer tissues. Expression of VDR in adjacent normal gastric mucosa and precancerous gastric mucosa and its clinical significance were analyzed. Materials and methods Immunohistochemical method was used to detect the expression of VDR in different types of gastric mucosal tissues, and chi-square test was used to detect the difference between the expression of VDR and the clinical parameters of patients with gastric cancer. Gastric cancer tissues and adjacent normal gastric mucosa tissues were collected from 92 patients undergoing radical gastrectomy in Gansu Provincial people's Hospital. The precancerous lesions included chronic atrophic gastritis (50 cases) and intestinal metaplasia (52 cases). Atypical hyperplasia (46 cases) was collected in endoscopic center of Gansu Provincial people's Hospital. Results VDR was mainly expressed in the cytoplasm of gastric mucosa, but not in the nucleus. Compared with normal gastric mucosa (82.61%) and precancerous gastric mucosa (73.64%), the expression of), VDR in gastric cancer tissues was lower (57.61%), with significant statistical difference (P0. 001).
