【摘要】：研究目的:研制簡(jiǎn)便、實(shí)用的中成藥參芪二仙顆粒,通過隨機(jī)對(duì)照的臨床研究驗(yàn)證其療效,發(fā)揮中醫(yī)藥治療肺癌化療相關(guān)性疲乏的優(yōu)勢(shì)。研究方法:在中醫(yī)理論指導(dǎo)下,結(jié)合科室臨床使用經(jīng)驗(yàn),將科室協(xié)定處方參芪二仙湯按照標(biāo)準(zhǔn)的工藝加工研制為參芪二仙顆粒。納入以肺癌化療后相關(guān)性疲乏為主要癥狀的患者76例,采用隨機(jī)的方法分為治療組(化療及基礎(chǔ)治療+參芪二仙顆粒)和對(duì)照組(單純化療及基礎(chǔ)治療),觀察參芪二仙顆粒對(duì)患者化療后疲乏程度、中醫(yī)證候積分、生存質(zhì)量等的影響。采用SPSS17.0進(jìn)行統(tǒng)計(jì)分析,其中計(jì)數(shù)資料按卡方檢驗(yàn)、計(jì)量資料按t檢驗(yàn)分析,檢驗(yàn)水準(zhǔn)為P0.05。研究結(jié)果:1、治療前兩組患者疲乏程度量表得分、中醫(yī)癥候積分、生存質(zhì)量得分差異無統(tǒng)計(jì)學(xué)意義(P0.05)。經(jīng)治療后治療組疲乏量表總分及各維度得分均降低,除認(rèn)知維度外,情感維度、軀體維度疲乏差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。中醫(yī)證后積分下降具有統(tǒng)計(jì)學(xué)意義(P0.05)。生存質(zhì)量評(píng)分有所提高,除社會(huì)狀況維度外,其余維度差異均具有統(tǒng)計(jì)學(xué)意義(P0.05)。2、治療組白細(xì)胞、淋巴細(xì)胞、血小板及血紅蛋白水平有所改善,其中白細(xì)胞、淋巴細(xì)胞和血紅蛋白具有統(tǒng)計(jì)學(xué)意義(P0.05)。對(duì)照組出現(xiàn)骨髓抑制32例,治療組出現(xiàn)骨髓抑制21例,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。3、對(duì)照組化療過程中出現(xiàn)胃腸道反應(yīng)15例,治療組共見12例,組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。研究結(jié)論:參芪二仙顆粒對(duì)肺癌患者化療相關(guān)性疲乏的改善具有確切療效,可以提高患者生存質(zhì)量,具有臨床推廣應(yīng)用的價(jià)值。
[Abstract]:Objective: to develop a simple and practical Chinese patent medicine Shenqi Erxian granule. The therapeutic effect of Shenqi Erxian granule was verified by randomized controlled clinical study to give full play to the advantage of traditional Chinese medicine in the treatment of lung cancer chemotherapy-related fatigue. Methods: under the guidance of the theory of traditional Chinese medicine and the clinical experience of the department, Shenqi Erxian decoction was prepared into Shenqi Erxian granule according to the standard technology. A total of 76 patients with lung cancer associated fatigue after chemotherapy were included. The patients were randomly divided into treatment group (chemotherapy and basic treatment) and control group (simple chemotherapy and basic treatment). The fatigue degree of Shenqi Erxian granule after chemotherapy and TCM syndromes score were observed. The influence of quality of life, etc. The statistical analysis was carried out by SPSS17.0, in which the counting data were checked by chi-square test, and the measured data were analyzed by t-test. The test level was P0.05. Results: 1, before the treatment of the two groups of patients fatigue scale score, TCM symptom score, quality of life score no significant difference (P0.05). After treatment the total score of fatigue scale and the scores of each dimension were decreased except cognitive dimension emotional dimension and somatic dimension fatigue difference was statistically significant (P0.05). After TCM syndromes score decreased statistically significant (P0.05). The scores of quality of life were improved, except the social status dimension, the differences of other dimensions were statistically significant (P0.05). 2. The levels of white blood cells, lymphocytes, platelets and hemoglobin were improved in the treatment group, in which white blood cells, blood platelets and hemoglobin were improved. Lymphocyte and hemoglobin had statistical significance (P0.05). There were 32 cases of bone marrow suppression in the control group and 21 cases in the treatment group, the difference was statistically significant (P0.05). In the control group, gastrointestinal reaction occurred in 15 cases during chemotherapy, and 12 cases in the treatment group. There was no significant difference between the two groups (P0.05). Conclusion: Shenqi Erxian granule has definite curative effect on the improvement of chemotherapy-related fatigue in lung cancer patients, it can improve the quality of life of patients, and has the value of clinical popularization and application.
