【摘要】：[目的]應(yīng)用分類樹(shù)模型構(gòu)建兒童急性淋巴細(xì)胞白血病(ALL)的影響因素模型,篩選環(huán)境危險(xiǎn)因素,為預(yù)防兒童ALL提供科學(xué)依據(jù)。[方法]采用病例-對(duì)照研究的流行病學(xué)方法,通過(guò)問(wèn)卷回顧性調(diào)查315名研究對(duì)象(兒童ALL患者為病例組,共179名;社會(huì)健康兒童為對(duì)照組,共136名)的相關(guān)信息,包含可能的兒童ALL影響因素30個(gè)。利用分類樹(shù)模型卡方自動(dòng)交互檢測(cè)(CHAID)法建立兒童ALL影響因素模型,通過(guò)收益圖、索引圖及受試者工作特征(ROC)曲線評(píng)價(jià)模型的應(yīng)用價(jià)值。[結(jié)果]分類樹(shù)模型包括4層,共11個(gè)節(jié)點(diǎn),篩選出5個(gè)兒童ALL的解釋變量,分別是:兒童不愛(ài)吃蔬菜水果(χ2=47.070,P0.001)、母親孕期服用藥物(χ2=13.638,P0.001)、母親孕期感冒(χ2=8.650,P=0.003)、兒童接觸油漆涂料(χ2=8.403,P=0.004)、母親孕期二手煙暴露(χ2=8.803,P=0.003)。模型的ROC曲線下面積為0.781,與曲線下面積0.5相比,差異有統(tǒng)計(jì)學(xué)意義(P0.001),模型的擬合效果較好。[結(jié)論]部分環(huán)境因素暴露可能與兒童ALL存在關(guān)聯(lián)。
[Abstract]:[objective] to construct a model of influencing factors of acute lymphoblastic leukemia (ALL) in children with acute lymphoblastic leukemia (AML) by using the classification tree model, and to screen the environmental risk factors and provide scientific basis for the prevention of ALL in children. [methods] the information of 315 subjects (179 children with ALL and 136 healthy children as control group) was investigated retrospectively by using the epidemiological method of case-control study. There were 30 possible factors affecting ALL in children. The influence factor model of child ALL was established by using the classification tree model chi-square automatic interactive detection (CHAID) method. The application value of the model was evaluated by income graph index graph and (ROC) curve of operating characteristics of subjects. [results] the classification tree model consisted of 4 layers and 11 nodes, and five explanatory variables of child ALL were screened out. The results were as follows: children did not like to eat fruits and vegetables (蠂 2 + 47.070), mothers took drugs during pregnancy (蠂 2 + 13.638% P 0.001), mothers had colds during pregnancy (蠂 2 + 8.650), children were exposed to paint (蠂 2) (蠂 2 + 8.403), and mothers were exposed to secondhand smoke during pregnancy (蠂 2 8. 803 P + 0.003). The area under the ROC curve of the model was 0.781.The difference was statistically significant compared with the area under the curve (P0. 001), and the fitting effect of the model was better. [conclusion] partial exposure to environmental factors may be associated with ALL in children.
