【摘要】：目的探討淋巴結轉移度(LNR)對老年結直腸癌患者術后遠處轉移的預測價值。方法選取2011年1月至2015年1月于該院普通外科行根治性手術的老年結直腸癌患者96例,按照有無術后遠處轉移將入組患者分為實驗組(轉移組,36例)和對照組(非轉移組,60例),按照LNR水平高低將入組患者分為HLNR組和LLNR組,統(tǒng)計分析LNR的相關臨床病理因素,單因素及多因素分析影響老年結直腸癌出現(xiàn)術后遠處轉移的臨床病理因素,總結分析LNR與老年結直腸癌患者出現(xiàn)術后遠處轉移相關性。結果 HLNR組和低LLNR組在年齡、大體類型和分化程度等方面均有統(tǒng)計學差異(均P0.05);Logistic多因素分析結果顯示,大體類型及分化程度與老年結直腸癌LNR水平獨立相關(均P0.05)。實驗組和對照組在大體類型、分化程度、淋巴結檢出總數(shù)(ELN)及LNR等方面均有統(tǒng)計學差異(均P0.05),Logistic多因素分析結果顯示:LNR、分化程度及大體類型與老年結直腸癌患者術后遠處轉移獨立相關(RR:2.098;CI%:1.050~4.192;P0.05)。結論 LNR、分化程度及大體類型與老年結直腸癌術后遠處轉移獨立相關,可聯(lián)合預測老年結直腸癌術后遠處轉移情況。
[Abstract]:Objective to evaluate the value of lymph node metastasis (LNR) in predicting distant metastasis after operation in elderly patients with colorectal cancer. Methods from January 2011 to January 2015, 96 elderly patients with colorectal cancer underwent radical surgery in general surgery in our hospital. The patients were divided into experimental group (metastatic group, 36 cases) and control group (non-metastatic group, 60 cases) according to the distant metastasis after operation. According to the level of LNR, the patients were divided into HLNR group and LLNR group. The clinicopathological factors related to LNR were statistically analyzed. Univariate and multivariate analysis were used to analyze the clinicopathological factors influencing the distant metastasis of elderly colorectal cancer, and to summarize and analyze the correlation between LNR and postoperative distant metastasis in elderly patients with colorectal cancer. Results there were significant differences in age, gross type and differentiation between HLNR group and low LLNR group (P0.05); Logistic multivariate analysis showed that gross type and differentiation degree were independently correlated with LNR level in elderly colorectal cancer (P0.05). Experimental group and control group in gross type, differentiation degree, The total number of lymph node detected (ELN) and LNR were significantly different (P0.05), Logistic multivariate analysis results showed that the degree of LNR, differentiation and gross type of colorectal cancer patients in the elderly after the distant metastasis (RR:2.098;CI%:1.050~4.192;P0.05) was independent correlation. Conclusion the degree of differentiation and gross type of LNR, are independently correlated with distant metastasis after operation of colorectal cancer, which can be combined to predict distant metastasis after operation of colorectal cancer in elderly patients.
