【摘要】：目的:總結、探討無功能性胰腺神經內分泌腫瘤的診斷和外科治療特點,進一步加深對該病的認識。方法:回顧性分析2002年1月至2014年12月福建醫(yī)科大學附屬第一醫(yī)院收治的16例無功能性胰腺神經內分泌腫瘤患者的臨床資料,分析、總結其臨床特點。結果:43.8%體檢發(fā)現(xiàn),出現(xiàn)臨床癥狀中:50.0%腹部悶痛、25.0%腹部包塊;92.3%初診BMI正�；虺�。檢測AFP、CEA、CA125、CA199均陰性。腫瘤93.8%單發(fā)、6.2%多發(fā)(胰頭1個、胰體1個),最大直徑平均5.16cm,94.1%≥2.0cm、52.9%≥5.0cm;41.2%合并囊性變,23.5%合并鈣化;部位:胰頭47.1%、胰體35.3%、胰尾17.6%。B超、CT、MRI檢出率均為100%;9例行B超檢查,表現(xiàn):77.8%界清,88.9%低回聲;15例行CT檢查,表現(xiàn):62.5%界清,平掃93.8%低密度,增強86.7%明顯強化,其中86.7%不均勻強化;4例行MRI檢查,表現(xiàn):平掃75.0%T1WI低信號、100%T2WI高信號,增強100%明顯強化。CT掃描聯(lián)合鈣化表現(xiàn)、CT掃描聯(lián)合囊性變表現(xiàn)、CT掃描聯(lián)合鈣化與囊性變表現(xiàn)、聯(lián)合CT平掃低密度與增強明顯強化表現(xiàn)、聯(lián)合B超低回聲與CT平掃低密度增強明顯強化表現(xiàn)者可降低誤診率。13行手術切除,3例非手術治療;15例獲得隨訪,早期患者、術后患者預后較好。結論:無功能性胰腺神經內分泌腫瘤臨床癥狀不典型,早期診斷困難,但影像學表現(xiàn)有特異之處;早期手術切除預后良好。
[Abstract]:Objective: to investigate the diagnosis and surgical treatment of nonfunctional neuroendocrine tumors of pancreas. Methods: the clinical data of 16 patients with nonfunctional pancreatic neuroendocrine tumors admitted to the first affiliated Hospital of Fujian Medical University from January 2002 to December 2014 were retrospectively analyzed and their clinical characteristics were summarized. Results: 43.8% of the clinical symptoms were found: 50.0% abdominal pain, 25.0% abdominal mass and 92.3% BMI were normal or overweight.
