【摘要】：背景:既往研究表明,在早期濾泡細(xì)胞淋巴瘤(FL)患者中初診時接受嚴(yán)格分期的患者,其預(yù)后明顯優(yōu)于非嚴(yán)格分期的患者,在國內(nèi)尚未見相關(guān)報(bào)道。目的:探討初診時嚴(yán)格分期與各期FL患者預(yù)后之間的關(guān)系。方法:收集和分析2008年到2014年共111名初診FL患者的臨床資料,其中Ⅰ-Ⅱ期患者42名,Ⅲ-Ⅳ期患者69名。嚴(yán)格分期定義為必須滿足以下2個必要條件:①骨髓穿刺和骨髓活檢;②全身影像學(xué)檢查(涵蓋全身的CT和超聲、PET/CT,至少滿足1項(xiàng))。檢查不滿足任1項(xiàng)即視為非嚴(yán)格的分期。結(jié)果:對早期的FL患者,接受嚴(yán)格分期患者的PFS明顯優(yōu)于未經(jīng)嚴(yán)格分期者(P=0.048)。針對各期FL患者的單因素分析顯示,年齡60歲、LDH升高、骨髓受累、大于3 cm病灶多于3處者,均與不良預(yù)后相關(guān)。多因素分析表明,年齡、LDH水平、骨髓受累為獨(dú)立預(yù)后因素。結(jié)論:對于初治的早期FL患者,接受嚴(yán)格分期者較未接受嚴(yán)格分期者有更好的預(yù)后。骨髓是否受累與預(yù)后密切相關(guān),臨床工作中應(yīng)重視骨髓狀況的評價。
