不同證型、病理分期的大腸癌癌灶及癌旁不同部位組織上細胞極性相關分子表達分析

【摘要】:目的:在結直腸癌高發(fā)病率的嚴峻形勢下,我國針對大腸癌的診療水平隨應上升,尤其中醫(yī)藥在診治腸癌的作用日益顯彰。筆者通過比較分析大腸癌患者不同證型(虛、實)與腫瘤病理分期,以及腸上皮細胞極性因子(Hugl-1/Podocalyxin)在虛實不同證型、不同病理分期及癌癥、癌旁不同距離組織的表達特征,探討人體不同證候的大環(huán)境下對大腸癌腫瘤微環(huán)境的影響,對腫瘤發(fā)生發(fā)展乃至轉(zhuǎn)移的影響。從而可為我國南方大腸癌的中醫(yī)辨證論治提供有效的生物學基礎,或許能針對早中晚期不同階段的大腸癌患者施予精準的中西醫(yī)診療方案,達到提高大腸癌患者生活質(zhì)量及生存率的最終目的。研究方法:本研究收集41例結直腸癌患者的手術標本(無淋巴轉(zhuǎn)移及遠處轉(zhuǎn)移的非轉(zhuǎn)移組23例,有淋巴轉(zhuǎn)移或者遠處轉(zhuǎn)移的轉(zhuǎn)移組18例),臨床研究部分:通過分析不同癥狀(虛、實)與不同病理分期(Ⅰ/Ⅱ、Ⅲ/Ⅳ期)的關系,尋找其中的聯(lián)系。實驗研究部分:在手術切除標本上癌灶、近端距離癌灶5cm、近端距離癌灶8-10cm取組織樣本,并依次命名為1、2、3號位樣本,進行石蠟切片、HE染色及免疫組化,觀察不同證型、不同分期、不同部位的腸上皮細胞極性分子Hugl-1蛋白及Podocalyxin蛋白的表達規(guī)律及差異。最后將圖片數(shù)據(jù)化,通過統(tǒng)計學探討其中的規(guī)律及意義。結果:1.術后病理提示癌細胞發(fā)生淋巴或遠處轉(zhuǎn)移的病例中以虛證為主(78%),而病理分期早的患者中虛證與實證所占的比例幾乎沒有差別,但虛實癥候與腫瘤分期沒有存在必然的聯(lián)系(P=0.05);2.Hugl-1蛋白表達水平與癌細胞是否發(fā)生淋巴或者遠處轉(zhuǎn)移呈顯著的負相關,具有統(tǒng)計學意義(P0.05),虛證組中癌灶(1號部位)Hugl-1蛋白表達量明顯低于實證組,具有統(tǒng)計學意義(P0.05);3.Podocalyxin蛋白表達水平與癌細胞是否轉(zhuǎn)移呈顯著的正相關,具有統(tǒng)計學意義(P0.05),虛證組中癌灶(1號部位)Hugl-1蛋白表達量明顯高于實證組,具有統(tǒng)計學意義(P0.05)。結論:1.正氣不足與大腸癌癌細胞發(fā)生淋巴或者遠處轉(zhuǎn)移具有一定的相關性;2.Hugl-1蛋白在大腸癌發(fā)生發(fā)展乃至轉(zhuǎn)移過程中起到抑制作用;3.Podocalyxin蛋白表達異常促使大腸癌疾病的發(fā)生并促進癌細胞遷移。
[Abstract]:Objective: in the severe situation of high incidence of colorectal cancer, the level of diagnosis and treatment of colorectal cancer in China should rise, especially the role of traditional Chinese medicine in the diagnosis and treatment of colorectal cancer.
