胃腺癌患者外周血SDF-1α含量表達檢測及與臨床病理特征的關(guān)系

【摘要】:目的:檢測和比較胃腺癌患者血清中SDF-1α蛋白含量的變化及其臨床意義。資料與方法:應(yīng)用酶聯(lián)免疫吸附測定法檢測90例胃腺癌患者以及25例健康志愿者血清中SDF-1α含量,單因素分析其與各項臨床病理特征及遠處轉(zhuǎn)移(術(shù)前常規(guī)行胸部及腹部增強CT檢測評估患者術(shù)前腫瘤轉(zhuǎn)移情況)特征的關(guān)系,進而行Logistic回歸分析SDF-1α高表達的獨立危險因子。Kaplan-Meier法繪制胃腺癌患者生存曲線,采用Log-rank法行單因素生存分析,Cox比例風(fēng)險回歸模型分析影響預(yù)后的獨立危險因素。結(jié)果:胃腺癌患者(n=90)血清SDF-1α含量[(6950.8±1131.3)pg/ml]明顯高于健康志愿者組成的正常對照組(n=25)[(5023.7±1103.8)pg/ml,(P=0.036)]。其中發(fā)生遠處轉(zhuǎn)移的胃腺癌患者(n=12)血清SDF-1α含量[(8251.6±1042.5)pg/ml]顯著高于未發(fā)生轉(zhuǎn)移患者(n=78)[(6785.3±1025.0)pg/ml,(P0.001)]。胃腺癌患者血清SDF-1α高含量與腫瘤分期較晚(P0.001)、淋巴結(jié)轉(zhuǎn)移更為廣泛(P=0.018)、腫瘤浸潤較深(P0.001)、存在血管侵犯(P0.001)和淋巴管侵犯(P0.001)有關(guān)。發(fā)生肝轉(zhuǎn)移(P=0.002)和肺轉(zhuǎn)移(P=0.030)的患者外周血SDF-1α含量高于未發(fā)生轉(zhuǎn)移的患者。Logistic回歸分析提示腫瘤浸潤深度是血清SDF-1α高含量的獨立危險因子(P=0.027)。外周血SDF-1α高含量患者術(shù)后生存時間顯著短于低含量患者(P0.001)。Cox比例風(fēng)險回歸模型分析提示腫瘤分期(P=0.009)、血管侵犯程度(P=0.002)及外周血SDF-1α含量(P=0.001)是胃癌患者預(yù)后的獨立危險因素。結(jié)論:胃腺癌患者外周血SDF-1α含量的升高或提示淋巴結(jié)、肝及肺轉(zhuǎn)移可能,并與胃癌的預(yù)后密切相關(guān)。
[Abstract]:Objective: to detect and compare the changes of serum SDF-1 偽 protein in patients with gastric adenocarcinoma and its clinical significance. Materials and methods: the serum levels of SDF-1 偽 in 90 patients with gastric adenocarcinoma and 25 healthy volunteers were detected by enzyme-linked immunosorbent assay (Elisa). Univariate analysis was used to analyze the relationship between the clinicopathological features and distant metastasis (preoperative routine chest and abdominal enhanced CT examination to evaluate preoperative tumor metastasis). Furthermore, the survival curve of gastric adenocarcinoma patients was drawn by logistic regression analysis with independent risk factor. Kaplan-Meier method. Single factor survival analysis was performed by Log-rank method and Cox proportional risk regression model was used to analyze the independent risk factors affecting prognosis. Results: the level of serum SDF-1 偽 in patients with gastric adenocarcinoma [(6950.8 鹵1131.3) pg/ml] was significantly higher than that in healthy volunteers (n = 25) [(5023.7 鹵1103.8) PG / ml, (P0. 036)]. The level of serum SDF-1 偽 in patients with distant metastasis (NM12) [(8251.6 鹵1042.5) pg/ml] was significantly higher than that in patients without metastasis (n = 78) [(6785.3 鹵1025.0) PG / ml, (P0.001)]. The high level of serum SDF-1 偽 in patients with gastric adenocarcinoma was related to the later stage (P0.001), the more extensive lymph node metastasis (P0. 018), the deeper invasion of tumor (P0. 001), the presence of vascular invasion (P0. 001) and lymphatic invasion (P0. 001). The level of SDF-1 偽 in peripheral blood of patients with liver metastasis (P0. 002) and pulmonary metastasis (P0. 030) was higher than that of patients without metastasis. Logistic regression analysis showed that the depth of tumor invasion was an independent risk factor for the high level of serum SDF-1 偽 (P0. 027). The survival time of patients with high level of peripheral blood SDF-1 偽 was significantly shorter than that of patients with low level of SDF-1 偽 (P0.001). Cox proportional risk regression model showed that tumor stage (P0. 009), degree of vascular invasion (P0. 002) and peripheral blood SDF-1 偽 content (P0. 001) were independent risk factors for prognosis of gastric cancer patients. Conclusion: the increase of SDF-1 偽 level in peripheral blood of patients with gastric adenocarcinoma suggests that lymph nodes, liver and lung metastasis may be associated with the prognosis of gastric cancer.
