【摘要】：背景與目的本研究旨在分析甲狀腺轉(zhuǎn)錄因子-1(thyroid transcription factor-1,TTF-1)、神經(jīng)細(xì)胞粘附分子CD56和P40蛋白在小細(xì)胞肺癌患者中的陽性表達(dá)情況,探討上述免疫表型標(biāo)志物及其他臨床特征與小細(xì)胞肺癌預(yù)后的相關(guān)性。方法用免疫組織化學(xué)方法檢測(cè)198例初次診治的小細(xì)胞肺癌患者石蠟包埋活檢組織標(biāo)本中TTF-1、CD56、P40的陽性表達(dá)情況,觀察隨訪患者臨床特征及治療、生存情況,通過Cox風(fēng)險(xiǎn)比例模型分析上述標(biāo)志物、臨床病理特征與預(yù)后的相關(guān)性。結(jié)果 198例小細(xì)胞肺癌患者TTF-1、CD56、P40的陽性率分別為73.2%(145/198)、88.4%(175/198)、7.1%(14/198)。TTF-1是否陽性為小細(xì)胞肺癌患者獨(dú)立預(yù)后因素OR=0.665,95%CI:0.472-0.937。其他與預(yù)后的相關(guān)因素包括:與不吸煙者相比,吸煙指數(shù)≤400組OR=1.72,95%CI:1.061-2.789;美國東部腫瘤協(xié)作組(Eastern Cooperative Oncology Group,ECOG)得分為2分與0分者相比的OR=3.551,95%CI:2.133-5.914;廣泛期與局限期患者相比OR=2.487,95%CI:1.793-3.451;合并上腔靜脈壓迫綜合征(superior vena cava syndrome,SVCS)者OR=2.394,95%CI:1.49-3.846。結(jié)論小細(xì)胞肺癌中的預(yù)后與TTF-1表達(dá)及吸煙、ECOG得分、腫瘤分期、合并SVCS等多個(gè)因素相關(guān),TTF-1、CD56、P40表達(dá)在小細(xì)胞肺癌的診斷和鑒別診斷中有輔助作用。
[Abstract]:Background & AIM this study was designed to analyze the positive expression of thyroid transcription factor 1 (thyroid transcription factor-1 TTF-1, neural cell adhesion molecule CD56 and P40 protein in patients with small cell lung cancer. To investigate the correlation between the immunophenotypic markers and other clinical features and the prognosis of small cell lung cancer. Methods Immunohistochemical method was used to detect the positive expression of TTF-1 CD56 P40 in paraffin-embedded biopsy specimens of 198 patients with primary small cell lung cancer. The clinical features, treatment and survival of the patients were observed. Cox risk ratio model was used to analyze the correlation between the clinicopathological features and prognosis. Results in 198 patients with small cell lung cancer, the positive rate of TTF-1 CD56 / P40 was 73.2% (14.5 / 198) and 88.4% (175 / 198) respectively. Whether TTF-1 was positive or not was the independent prognostic factor of small cell lung cancer patients: CIW 0.66595and CIW 0.472-0.937.
