【摘要】：[目的]對比分析3D與2D腹腔鏡在直腸癌根治術(shù)中的優(yōu)勢和近期臨床療效。[方法]收集2015年1月至2016年3月我科收治的100例直腸癌患者的臨床資料,按照入院的前后順序編為1-100號,再使用隨機(jī)數(shù)字表給每位患者分配一個隨機(jī)數(shù),按照隨機(jī)數(shù)大小由小到大對患者編號進(jìn)行排序,排序號1-50為3D組,采用3D腹腔鏡進(jìn)行手術(shù)。51-100為2D組,采用2D腹腔鏡進(jìn)行手術(shù)。對比分析兩組患者的術(shù)中、術(shù)后情況以及病理學(xué)標(biāo)本評價。應(yīng)用SPSS 22.0進(jìn)行統(tǒng)計學(xué)分析,計量資料以x±s表示,計量資料采用t檢驗,計數(shù)資料采用x~2檢驗。[結(jié)果]兩組患者基線資料無統(tǒng)計學(xué)意義,具有可比性。3D腹腔鏡組比2D腹腔鏡組的手術(shù)時間短[(91.8±26.1)min比(100.3±32.7)min,t=-1.439,P=0.038)]、術(shù)中出血量少[(222.4±73.0)比(302.0±92.4)ml,t=-4.780,P= 0.024)],在術(shù)后排氣時間、術(shù)后住院時間、術(shù)后并發(fā)癥(肺部感染、切口感染、吻合口瘺、腸梗阻、尿儲留)、非計劃二次手術(shù)、近期生存率(12月)、術(shù)后病理標(biāo)本評價這些方面兩組間比較無統(tǒng)計學(xué)意義(P0.05)。[結(jié)論]與2D腹腔鏡相比,3D腹腔鏡在直腸癌根治術(shù)中具有加快手術(shù)進(jìn)程,減少術(shù)中出血量,提高術(shù)中操作精準(zhǔn)度,減少無效操作的優(yōu)點。近期臨床療效無明顯差異。
[Abstract]:[objective] to compare and analyze the advantages and short-term clinical effects of 3 D and 2 D laparoscopy in radical resection of rectal cancer. [methods] the clinical data of 100 patients with rectal cancer admitted from January 2015 to March 2016 were collected. According to the order of admission, they were numbered 1-100, and each patient was assigned a random number by random number table. According to the size of random numbers, the patient numbers were sorted from small to large. The sequence numbers were 1-50 for 3D group, 51-100 for 2D group, and 2-D for laparoscopy. The intraoperative, postoperative and pathological evaluation of the patients in the two groups were compared and analyzed. The statistical analysis was carried out with SPSS 22.0. The metrological data were expressed as x 鹵s, the measurement data were t test, and the counting data were x + 2 test. [results] there was no statistical significance in baseline data between the two groups. The operative time in the 3D laparoscopic group was shorter than that in the 2D laparoscopic group [91.8 鹵32.7 26.1)min vs 100.3 鹵32.7 mint-1.439m P0.038], and the amount of intraoperative bleeding was less [222.4 鹵73.0 vs 302.0 鹵92.4ml / t -4.780P = 0.024]. Postoperative complications (pulmonary infection, incision infection, anastomotic fistula, intestinal obstruction, urinary retention, unplanned secondary surgery, short term survival rate) were not statistically significant (P 0.05). [conclusion] compared with 2D laparoscopy, 3D laparoscopy has the advantages of speeding up the operation process, reducing the amount of blood loss, improving the accuracy of operation and reducing the invalid operation in the radical resection of rectal cancer. There was no significant difference in clinical efficacy in the near future.
