HCRP1基因在胃癌中的表達及與胃癌臨床病理特征的相關(guān)性研究

【摘要】:目的研究胃癌組織中HCRP1基因的表達情況,并結(jié)合所收集患者的臨床病理特征及相關(guān)資料,分析HCRP1基因的不同表達與患者臨床病理特征之間的相關(guān)性,以及HCRP1基因的不同表達對胃癌患者生存預(yù)后的影響。方法收集胃癌病人手術(shù)切除的癌旁正常組織及胃癌組織,應(yīng)用免疫組織化學(xué)染色方法檢測所有癌旁正常組織及胃癌組織中HCRP1蛋白的表達情況,應(yīng)用逆轉(zhuǎn)錄一聚合酶鏈式反應(yīng)(RT-PCR)方法檢測所有癌旁正常組織及胃癌組織中HCRP1mRNA的表達情況。并應(yīng)用單因素方差分析HCRP1基因表達的影響因素。同時應(yīng)用Kaplan-Meier生存分析法分析HCRP1蛋白的不同表達與患者遠期生存的關(guān)系,COX回歸模型分析患者預(yù)后的獨立影響因素。結(jié)果免疫組化染色結(jié)果顯示HCRP1蛋白在癌旁正常胃粘膜組織中的表達量要高于相鄰胃癌組織中的表達量,且差異具有統(tǒng)計學(xué)意義,但HCRP1蛋白在不同分化程度胃癌組織中的表達差異無統(tǒng)計學(xué)意義。單因素方差分析發(fā)現(xiàn)HCRP1蛋白的不同表達與患者的淋巴結(jié)轉(zhuǎn)移、遠處轉(zhuǎn)移、TNM分期以及HCRP1蛋白表達有關(guān)。RT-PCR結(jié)果顯示HCRP1mRNA在癌旁正常胃粘膜組織中高表達,而在胃癌組織中呈現(xiàn)低表達,差異具有統(tǒng)計學(xué)意義。并且通過RT-PCR檢測發(fā)現(xiàn)不同分化程度的胃癌組織中HCRP1表達量不同,但差異無統(tǒng)計學(xué)意義。Kaplan-Meier生存分析顯示HCRP1蛋白高表達的胃癌患者其遠期生存率要遠遠高于HCRP1蛋白低表達的胃癌患者(P0.05),單因素生存分析發(fā)現(xiàn)HCRP1蛋白表達、淋巴結(jié)轉(zhuǎn)移、遠處轉(zhuǎn)移及TNM分期是預(yù)后的影響因素。多因素COX分析發(fā)現(xiàn)遠處轉(zhuǎn)移和TNM分期是預(yù)后的獨立影響因素。結(jié)論HCRP1基因可以抑制胃癌的生長及增值,其在癌旁正常胃粘膜組織中高表達,在胃癌組織中低表達。HCRP1基因表達與胃癌患者是否有淋巴結(jié)轉(zhuǎn)移、遠處轉(zhuǎn)移以及TNM分期有關(guān),與患者的年齡、性別、腫塊大小和分化程度無關(guān)。并且HCRP1蛋白高表達的胃癌患者其預(yù)后要好于HCRP1蛋白低表達的胃癌患者。因此可將HCRP1基因作為一種預(yù)測胃癌患者術(shù)后生存的指標應(yīng)用于臨床,同時對于評估胃癌的臨床分期和指導(dǎo)其治療也有重要的意義。
[Abstract]:Objective to study the expression of HCRP1 gene in gastric cancer and analyze the correlation between the expression of HCRP1 gene and the clinicopathological features of gastric cancer. And the effect of different expression of HCRP1 gene on survival and prognosis of gastric cancer patients. Methods the expression of HCRP1 protein was detected by immunohistochemical staining in normal tissues and gastric cancer tissues of patients with gastric cancer. Reverse transcription-polymerase chain reaction (RT PCR) was used to detect the expression of HCRP1mRNA in all adjacent normal tissues and gastric cancer tissues. Univariate ANOVA was used to analyze the influencing factors of HCRP1 gene expression. At the same time, Kaplan-Meier survival analysis was used to analyze the relationship between the expression of HCRP1 protein and the long-term survival of patients. The Cox regression model was used to analyze the independent prognostic factors of patients. Results Immunohistochemical staining showed that the expression of HCRP1 protein in adjacent normal gastric mucosa was higher than that in adjacent gastric cancer tissues, and the difference was statistically significant. However, there was no significant difference in the expression of HCRP1 protein in different differentiated gastric cancer tissues. Univariate ANOVA analysis showed that different expression of HCRP1 protein was related to lymph node metastasis, distant metastasis stage and HCRP1 protein expression. RT-PCR results showed that HCRP1mRNA was highly expressed in adjacent normal gastric mucosa, but low in gastric cancer tissue. The difference is statistically significant. The expression of HCRP1 was found to be different in gastric cancer tissues with different differentiation degree by RT-PCR detection. However, Kaplan-Meier survival analysis showed that the long-term survival rate of patients with high expression of HCRP1 protein was much higher than that of patients with low expression of HCRP1 protein. Univariate survival analysis showed the expression of HCRP1 protein and lymph node metastasis. Distant metastasis and TNM stage were the prognostic factors. Multivariate COX analysis showed that distant metastasis and TNM staging were independent prognostic factors. Conclusion HCRP1 gene can inhibit the growth and proliferation of gastric cancer. It is highly expressed in normal gastric mucosa adjacent to gastric cancer. The low expression of .HCRP1 gene in gastric cancer is related to lymph node metastasis, distant metastasis and TNM staging. There was no correlation between age, sex, tumor size and differentiation. The prognosis of gastric cancer patients with high expression of HCRP1 protein was better than that with low expression of HCRP1 protein. Therefore, HCRP1 gene can be used as a predictor of postoperative survival in patients with gastric cancer. It is also of great significance to evaluate the clinical stage and guide the treatment of gastric cancer.
