【摘要】：目的：評估術后血清腫瘤標記物水平，對于胃癌術后復發(fā)轉移的診斷價值。 材料和方法：我們回顧性的分析了2007年9月－2012年9月就診于吉大一院胃腸外科的1651例胃癌患者，其中1480例患者接受根治性外科手術治療，170例患者術后2年內出現(xiàn)復發(fā)轉移，對術后血清腫瘤標記物水平與胃癌術后復發(fā)轉移的關系進行分析。 結果：血清腫瘤標記物CA199和CEA在5項腫瘤標記物中對于胃癌術后復發(fā)及轉移有較高的敏感性及特異性。其中CEA對于預測腫瘤復發(fā)敏感性及特異性分別為40.6%及89.5%，CA199對于預測腫瘤復發(fā)及轉移敏感性及特異性分別為34.2%及93.6%。通過調整CA199的臨界值，能提高腫瘤標記物對于術后腫瘤復發(fā)預測的敏感性。但調整CEA的臨界值值卻沒有相同的效果。 結論：在5種腫瘤標記物中，腫瘤標記物CEA及CA199對于胃癌術后復發(fā)轉移的預測有較高的特異性及敏感性，，通過調整CA199的臨界值能提高腫瘤標記物對于預測胃癌復發(fā)的敏感性。
[Abstract]:Objective: to evaluate the value of serum tumor markers in the diagnosis of postoperative recurrence and metastasis of gastric cancer. Materials and methods: we retrospectively analyzed 1651 patients with gastric cancer who were treated in gastrointestinal surgery from September 2007 to September 2012, 1480 of whom were treated with radical surgical treatment, 170 patients had recurrence and metastasis within 2 years after operation. The relationship between serum tumor markers and recurrence and metastasis of gastric cancer after operation was analyzed. Results: serum tumor markers CA199 and CEA had high sensitivity and specificity for recurrence and metastasis of gastric cancer. The sensitivity and specificity of CEA for predicting tumor recurrence were 40.6% and 89.5% respectively. The sensitivity and specificity of CA199 for predicting tumor recurrence and metastasis were 34.2% and 93.6g respectively. By adjusting the critical value of CA199, the sensitivity of tumor markers to the prediction of postoperative tumor recurrence can be improved. But adjusting the critical value of CEA does not have the same effect. Conclusion: among the five tumor markers, tumor markers CEA and CA199 have high specificity and sensitivity in predicting recurrence and metastasis of gastric cancer after operation. The sensitivity of tumor markers to predicting recurrence of gastric cancer can be improved by adjusting the critical value of CA199.
