腎透明細(xì)胞癌組織ICAM1表達(dá)臨床意義探討

【摘要】:目的細(xì)胞間黏附分子-1(intercellular cell adhesion molecule-1,ICAM1)在多種惡性腫瘤中都發(fā)揮著重要作用,但其對(duì)腎透明細(xì)胞癌(clear-cell renal cell carcinoma,ccRCC)的影響鮮見(jiàn)報(bào)道。本研究旨在探討ICAM1在ccRCC預(yù)后判斷以及早期診斷中的意義。方法隨機(jī)選擇2000-01-18-2010-09-08皖南醫(yī)學(xué)院弋磯山醫(yī)院ccRCC手術(shù)冷凍標(biāo)本173例,利用免疫組織化學(xué)(immunohistochemistry,IHC)方法檢測(cè)ICAM1在ccRCC中的表達(dá),并分析ICAM1表達(dá)與臨床病理參數(shù)的相關(guān)性。應(yīng)用生存分析研究ICAM1表達(dá)對(duì)ccRCC患者腫瘤特異生存率(cancer-specific survival,CSS)的影響。通過(guò)計(jì)算受試者工作特征曲線(receiver operating charccteristic curve,ROC)下的面積(area under the ROC curve,AUC)來(lái)評(píng)價(jià)ICAM1表達(dá)對(duì)ccRCC早期診斷的價(jià)值。結(jié)果173例ccRCC標(biāo)本組織中,ICAM1陽(yáng)性率為83.2%(144/173);173例對(duì)應(yīng)的癌旁正常腎小管上皮組織中,ICAM1均為陰性。ICAM1在男性(χ~2=9.208,P=0.002)、T3/T4期(χ~2=5.207,P=0.022)、非N0M0期(χ~2=4.306,P=0.038)以及腎盂侵犯(χ~2=4.124,P=0.042)等亞組中高表達(dá)率分別為56.9%(74/130)、65.9%(27/41)、75.0%(12/16)和67.9%(19/28),均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義。Kaplan-Meier生存分析提示,ICAM1高表達(dá)時(shí),ccRCC患者的CSS顯著下降(P0.001),5和10年累積CSS與ICAM1低表達(dá)患者相比,分別下降了21.6%和14.1%。Cox回歸分析提示,ICAM1是判斷ccRCC患者CSS的獨(dú)立預(yù)測(cè)因子,P=0.005。ICAM1在ccRCC早期診斷中的AUC值為0.916,P0.001。結(jié)論 ICAM1高表達(dá)時(shí),ccRCC患者預(yù)后差,ICAM1是判斷ccRCC患者預(yù)后的獨(dú)立預(yù)測(cè)因子,并且可能成為ccRCC早期診斷的潛在標(biāo)志物。
[Abstract]:Objective Intercellular cell adhesion molecule-1 (ICAM1) plays an important role in many kinds of malignant tumors, but its effect on clear cell renal cell carcinoma (CCCC) is rarely reported. The purpose of this study was to investigate the significance of ICAM1 in early diagnosis and prognosis of ccRCC. Methods 173 frozen specimens of ccRCC were randomly selected from Yaji Mountain Hospital of Southern Anhui Medical College from January 18 to August 2010. Immunohistochemistry was used to detect the expression of ICAM1 in ccRCC, and the correlation between the expression of ICAM1 and clinicopathological parameters was analyzed. Survival analysis was used to study the effect of ICAM1 expression on tumor-specific survival rate (ccRCC) in patients with ccRCC. The value of ICAM1 expression in the early diagnosis of ccRCC was evaluated by calculating the area under the ROC under receiver operating charccteristic curveout. 緇撴灉173渚媍cRCC鏍囨湰緇勭粐涓,
