【摘要】：目的:系統(tǒng)評價含鉑化療方案治療三陰性乳腺癌(TNBC)的臨床療效。方法:計算機檢索The PubMed、Embase、CBM、CNKI、VIP、Conchrane Library等國內(nèi)外重要的數(shù)據(jù)庫。納入含鉑類與非鉑類化療藥物治療TNBC的隨機對照試驗,檢索從建庫至2016年5月相關(guān)資料,同時查閱所獲取的參考文獻索引,并由兩位研究者對檢索收集到的文獻按照文章制定的納入、排除標(biāo)準(zhǔn)進行獨立篩選。采用RevMan5.3軟件進行系統(tǒng)評價。結(jié)果:最終納入10項研究,共TNBC患者1367例。Meta分析結(jié)果顯示:兩組在客觀緩解率(ORR)、病理完全緩解率(p CR)、血小板減少及貧血方面差異有統(tǒng)計學(xué)意義(P0.05);在總生存率(OS)、無進展生存率(PFS)、惡心嘔吐及白細(xì)胞減少方面差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:含鉑化療方案可提高TNBC患者ORR、p CR,但患者血小板減少、貧血發(fā)生率高于非含鉑化療方案。臨床選擇過程中需權(quán)衡其利弊。
[Abstract]:Objective: to evaluate the clinical efficacy of chemotherapy regimen containing platinum in the treatment of triple negative breast cancer. Methods: The PubMedus Embase Library and other important databases at home and abroad were searched by computer. A randomized controlled trial of platinum-containing and non-platinum chemotherapeutic drugs for treatment of TNBC was conducted to retrieve relevant data from the establishment of the database to May 2016, as well as the index of references obtained, Two researchers independently sifted the collected literature according to the inclusion and exclusion criteria. RevMan5.3 software was used to evaluate the system. Results: eventually included in 10 studies, A meta-analysis of 1367 patients with TNBC showed that there were significant differences in objective remission rate (ORR), pathological complete remission rate (CR), thrombocytopenia and anemia between the two groups (P < 0.05), and in overall survival rate, progressive survival rate (P < 0.05), nausea and vomiting, and white. There was no significant difference in cell reduction (P 0.05). Conclusion: platinum-containing chemotherapy regimen can increase ORRP-CRs in patients with TNBC, but the incidence of anemia in patients with thrombocytopenia is higher than that in non-platinum chemotherapy regimen. In the course of clinical selection, the pros and cons should be weighed.
