【摘要】：目的應(yīng)用雙源CT后處理技術(shù)計(jì)算肺癌患者吸氣末和呼氣末容量參數(shù),并與傳統(tǒng)肺功能檢查(PFT)參數(shù)相對(duì)比,探討其評(píng)價(jià)肺功能中的意義。方法對(duì)32例肺癌患者,術(shù)前1周內(nèi)行肺功能檢查及深吸氣相和深呼氣胸部CT掃描,采用SIEMENS工作站Volume及Pulmo模塊軟件進(jìn)行掃描后處理,得到吸氣末和呼氣末的肺容積。將CT后處理數(shù)據(jù)與肺功能檢查主要數(shù)據(jù)相比較,采用Pearson分析相關(guān)性。結(jié)果肺癌患者術(shù)前吸氣相和呼氣相全肺容積差值(Vin-Vex)與肺總量(TLC)、第1秒用力肺活量的實(shí)測(cè)值FEV1、用力肺活量實(shí)際值(FVC)呈正相關(guān)(r=0.80、0.91和0.82,均P0.05),CT掃描獲得的吸氣相容積值(Vin)與肺總量(TLC)呈正相關(guān)(r=0.91,P0.05)。呼氣相全肺容積值(Vex)與殘氣量(Rv)呈正相關(guān)(r=0.79,P0.05)。結(jié)論 CT后處理技術(shù)在肺癌圍手術(shù)期中的應(yīng)用,對(duì)肺癌患者肺功能的評(píng)估提供新的可能,可以作為常規(guī)肺功能檢查的補(bǔ)充。
[Abstract]:Objective to calculate the end-inspiratory and end-expiratory volume parameters of lung cancer patients by dual-source CT post-processing, and to evaluate the significance of the parameters compared with the traditional pulmonary function test (PFTs). Methods 32 patients with lung cancer were performed lung function examination and chest CT scan of deep inspiratory phase and deep expiratory chest within 1 week before operation. The lung volume at the end of inspiratory and end expiratory was obtained by SIEMENS workstation Volume and Pulmo module software. Ct postprocessing data were compared with the main data of pulmonary function examination, and correlation was analyzed by Pearson. Results preoperative inspiratory and expiratory lung volume differences (Vin-Vexx) were positively correlated with total lung volume (FEV1), forced vital capacity (FEV1) and forced vital capacity (FVC1) in patients with lung cancer. There was a positive correlation between TLC and TLC. There was a positive correlation between VX and RV in expiratory phase. Conclusion the application of CT post-processing technique in lung cancer perioperative period provides a new possibility for lung function evaluation in lung cancer patients and can be used as a supplement to routine lung function examination.
