【摘要】：目的:比較經直腸超聲引導下經直腸與經會陰前列腺穿刺活檢術的有效性及并發(fā)癥。材料與方法:隨機選取2013-2014年間于浙江大學附屬第一醫(yī)院行前列腺穿刺活檢術患者270例,通過本院病歷系統(tǒng)收集臨床資料并進行回顧性分析。結果:經直腸前列腺穿刺組穿刺陽性率為42.22%(57/135),經會陰前列腺穿刺組穿刺陽性率為44.44%(60/135)。兩組穿刺陽性率無統(tǒng)計學差異(P0.05)。不同PSA水平及年齡段亞組中經會陰前列腺穿刺組與經直腸前列腺穿刺組穿刺陽性率均無顯著差異。兩組檢出前列腺癌Gleason評分無顯著差異(P0.05)。經直腸前列腺穿刺組總穿刺并發(fā)癥發(fā)生率(27.41%)顯著高于經會陰前列腺穿刺組(8.89%,P0.01)。經直腸前列腺穿刺組(21.48%)顯著高于經直腸前列腺穿刺組血尿發(fā)生率(6.67%,P0.01)。其余并發(fā)癥無顯著差異。結論:經會陰前列腺穿刺與經直腸前列腺穿刺在前列腺癌檢出上無顯著差異,但經會陰前列腺穿刺并發(fā)癥顯著少于經直腸前列腺穿刺。
[Abstract]:Objective: to compare the effectiveness and complications of transrectal and transperineal prostatic biopsy guided by transrectal ultrasound. Materials and methods: 270 patients who underwent prostate biopsy in the first affiliated Hospital of Zhejiang University from 2013 to 2014 were selected randomly. The clinical data were collected and analyzed retrospectively. Results: the positive rate of transrectal prostate puncture group was 42.22% 57 / 135 and that of perineum prostate puncture group was 44.4460 / 135%. There was no significant difference in the positive rate of puncture between the two groups (P 0.05). There was no significant difference in the positive rate between the transperineal and transrectal prostatic puncture groups in different PSA levels and age subgroups. There was no significant difference in Gleason score between the two groups (P 0.05). The incidence of total puncture complications in the transrectal prostate puncture group (27.41) was significantly higher than that in the perineal prostate puncture group (8.89%). The incidence of hematuria in the transrectal prostate puncture group was significantly higher than that in the transrectal prostate puncture group (P 0.01). There was no significant difference in other complications. Conclusion: there is no significant difference between transperineal and transrectal prostate puncture in the detection of prostate cancer, but the complications of transperineal prostate puncture are significantly less than that of transrectal prostate puncture.
