【摘要】：針對CD19靶點的嵌合抗原受體(chimeric antigen receptor,CAR)T細胞在B細胞惡性血液系統(tǒng)腫瘤治療中取得的成功,證實了抗原特異性靶向免疫細胞對腫瘤治療的臨床價值。同時這一發(fā)現(xiàn)也激發(fā)了CAR-T細胞技術的拓展應用性研究,多項CART細胞治療實體瘤的臨床前研究和臨床研究正在開展。然而實體腫瘤和血液腫瘤本身特質(zhì)上的差異決定了這種治療模式必須重新斟酌細胞治療本身的特點、作用規(guī)律等因素。本文討論了CAR-T細胞治療實體瘤相關的安全性管控措施、腫瘤靶點選擇、預處理及聯(lián)合治療,并通過相關的實踐經(jīng)驗展望該治療方式的未來發(fā)展方向。
[Abstract]:The success of chimeric antigen receptor CART cells targeting CD19 targets in the treatment of malignant hematopoietic tumors of B cells has confirmed the clinical value of antigen-specific immunocytes in the treatment of tumors. At the same time, this discovery has stimulated the extensive application of CAR-T cell technology, and many preclinical and clinical studies of CART cell therapy for solid tumors are being carried out. However, the difference between solid tumor and blood tumor determines that this treatment mode must reconsider the characteristics and action rules of cell therapy itself. In this paper, the safety control measures, tumor target selection, preconditioning and combined therapy for solid tumor treated by CAR-T cells are discussed, and the future development of this treatment method is prospected through relevant practical experience.
