【摘要】：目的探討活絡(luò)效靈丹辨證輔助治療對中重度癌性疼痛的鎮(zhèn)痛效應(yīng)及機制研究。方法將206例患者分為對照組104例和觀察組102例。對照組,中度疼痛患者采用布洛芬片+鹽酸羥考酮緩釋片;重度疼痛采用布洛芬片+硫酸嗎啡緩釋片。觀察組在對照組的基礎(chǔ)上采用活絡(luò)效靈丹辨證。療程均為10d。采用疼痛視覺模擬評分(VAS)記錄患者疼痛情況;進行治療前后簡明疼痛評估量表和生活質(zhì)量評分;檢測治療前后P物質(zhì)(SP)、β-內(nèi)啡肽(β-EP)和前列腺素E2(PGE2)水平。結(jié)果觀察組疼痛緩解率為94.12%,高于對照組的81.73%(χ~2=7.409,P0.01);觀察組在第5~10天VAS評分均低于對照組(P0.01);觀察組疼痛最劇程度、疼痛最輕程度、平均疼痛程度和目前疼痛程度評分均低于對照組(P0.01);觀察組情緒等疼痛影響評分均低于對照組(P0.01);觀察組患者的生活質(zhì)量情況優(yōu)于對照組(P0.05);觀察組SP和PGE2低于對照組,β-EP水平高于對照組(P0.01)。結(jié)論活絡(luò)效靈丹辨證輔助治療中重度癌性疼痛,能減輕患者疼痛,改善疼痛導(dǎo)致的影響,提高患者的生活質(zhì)量,其作用機制可能與降低SP和PGE2,升高β-EP有關(guān)。
[Abstract]:Objective to investigate the analgesic effect and mechanism of the auxiliary treatment of activating collaterals and panacea to treat moderate to severe cancer pain. Methods 206 patients were divided into control group (104 cases) and observation group (102 cases). The control group was treated with Ibuprofen Tablets + Oxycodone Hydrochloride Prolonged-release Tablets for moderate pain and severe pain by Ibuprofen Tablets + Morphine Sulfate Sustained-release Tabelets. On the basis of the control group, the pain situation of the patients was recorded by 10d. with the visual analogue score (VAS); the simple pain assessment scale and the quality of life score before and after treatment; the level of P substance (SP), beta endorphin (beta -EP) and prostaglandin E2 (PGE2) before and after treatment. Results the pain relief rate of the observation group. 94.12%, higher than the control group of 81.73% (x ~2=7.409, P0.01), the observation group on day 5~10 VAS scores were lower than the control group (P0.01); the pain most of the observation group, the least pain degree, the average pain degree and the current pain degree scores were lower than the control group (P0.01); the observation group emotional pain impact scores were lower than the control group (P0.01); observation group (P0.01); observation group (P0.01); observation group The quality of life of the patients was better than that of the control group (P0.05), and the SP and PGE2 in the observation group were lower than the control group, and the level of beta -EP was higher than that of the control group (P0.01). Conclusion the syndrome differentiation assisted treatment for moderate and severe cancer pain can relieve the pain, improve the effect of pain and improve the quality of life of the patients. The mechanism of the action may be reduced by SP and PGE2, The increase of beta -EP is related.
