【摘要】：目的探討MRI對乳腺癌患者腋窩淋巴結轉移的鑒別診斷價值。方法分析經(jīng)病理證實并行腋窩MR掃描的44例乳腺癌患者資料,分析MRI表現(xiàn),包括淋巴結長徑、短徑、皮質厚度、ADC值、淋巴門情況、淋巴結邊緣、周圍脂肪間隙情況、DWI信號、強化方式及時間—信號強度曲線等,并繪制的ROC曲線分析淋巴結長徑、短徑、皮質厚度、ADC值對腋窩淋巴結轉移的診斷效能。結果病理結果顯示有淋巴結轉移者24例(24/44,54.55%),無淋巴結轉移者20例(20/44,45.45%),兩者淋巴結長徑、短徑、皮質厚度、ADC值、淋巴門是否消失、淋巴結邊緣、周圍脂肪間隙、DWI信號、強化方式差異均有統(tǒng)計學意義(P均0.05)。淋巴結長徑、短徑、皮質厚度及ADC值的ROC曲線下面積分別為0.797、0.765、0.848、0.749。結論 MRI在鑒別乳腺癌腋窩淋巴結狀態(tài)中有重要價值,皮質厚度大于0.54cm高度提示腋窩淋巴結轉移。
[Abstract]:Objective to evaluate the value of MRI in differential diagnosis of axillary lymph node metastasis in breast cancer. Methods the data of 44 patients with breast cancer confirmed by pathology and axillary Mr scan were analyzed. The MRI findings, including lymph node length, short diameter, cortical thickness, lymphatic hilum, lymph node margin and peripheral fat space, were analyzed. The enhancement mode and the time-signal intensity curve were used to analyze the diagnostic efficacy of the long diameter, short diameter and cortical thickness of lymph nodes in the diagnosis of axillary lymph node metastasis. Results the pathological results showed that 24 cases had lymph node metastasis (24 / 44) 54.55% and 20 cases had no lymph node metastasis (20% 44.45%). Both lymph nodes had long diameter, short diameter, cortical thickness and ADC value, lymphoid hilum disappeared, lymph node margin, peripheral fat space and DWI signal. There were significant differences in the reinforcement patterns (P < 0.05). The area under the ROC curve of long diameter, short diameter, cortical thickness and ADC value of lymph nodes were 0.7970.765U 0.848c0.749, respectively. Conclusion MRI plays an important role in differentiating axillary lymph node status of breast cancer. The thickness of cortex is higher than that of 0.54cm suggesting axillary lymph node metastasis.
