【摘要】：目的探討磷酸二酯酶4B(phosphdiesterase 4B,PDE4B)基因過表達對彌漫大B細胞淋巴瘤(diffuse large B-cell lymphoma,DLBCL)患者預后的影響,并分析其預后價值。方法從GEO(gene expression omnibus)數(shù)據(jù)庫獲得320例DLBCL患者的基因芯片數(shù)據(jù),比較PDE4B基因過表達與正常表達患者生存率的差異。構建單變量國際預后指數(shù)(international prognostic index,IPI)、PDE4B和多變量(IPI+PDE4B)Cox比例風險模型,并用時點/動態(tài)受試者工作曲線(receiver operating characteristic curves,ROC)法分析模型的預測能力隨時間的變化趨勢,一致性統(tǒng)計量C用于評價和比較各模型的預測準確度。結果 PDE4B基因過表達的患者死亡的風險是正常表達的1.431倍(HR=1.431,95%CI:1.013~2.002,P=0.042)。多變量模型的C統(tǒng)計量與單變量模型相比,差異有統(tǒng)計學意義(IPI vs.IPI+PDE4B:Z=15.0,P0.001;PDE4B vs.IPI+PDE4B:Z=53.7,P0.001)。結論 PDE4B基因過表達是DLBCL患者預后的危險因素,結合IPI和PDE4B基因能提高DLBCL的預后準確性。
[Abstract]:Objective to investigate the effect of the overexpression of phosphodiesterase 4B(phosphdiesterase 4BmP PDE4B gene on the prognosis of diffuse large B-cell lymphoma (DLBCL) patients with diffuse large B-cell lymphoma (DLBCL), and to analyze its prognostic value. Methods the microarray data of 320 patients with DLBCL were obtained from GEO(gene expression omnibus database. The survival rates of patients with PDE4B overexpression and normal expression were compared. The international prognostic index IPE4B and multivariate PDE4B)Cox proportional risk model were constructed. The prediction ability of the model was analyzed by using the time-point / dynamic receiver operating characteristic curvesroc method. Consistency statistic C is used to evaluate and compare the prediction accuracy of each model. Results the risk of death in patients with overexpression of PDE4B gene was 1.431 times higher than that in normal expression. Compared with the single variable model, the C statistic of multivariate model has statistical significance. The difference of IPI vs.IPI PDE4B: Z0. 001n PDE4B vs.IPI PDE4B: P0.001is significant. Conclusion overexpression of PDE4B gene is a risk factor for the prognosis of patients with DLBCL. The combination of IPI and PDE4B genes can improve the prognosis accuracy of DLBCL.
