DCLK1、Bmi-1在大腸癌中的表達(dá)及臨床意義

【摘要】:【目的】:本研究旨在聯(lián)合檢測(cè)DCLK1和Bmi-1在74例大腸癌組織中的表達(dá)狀況,了解DCLK1和Bmi-1與大腸癌之間的關(guān)系。【方法】:收集延安大學(xué)附屬醫(yī)院2015年9月到2016年10月74例大腸癌患者完整的病理資料、臨床資料,根治術(shù)前均未行放化療及其他相關(guān)治療,所選實(shí)驗(yàn)標(biāo)本包括大腸癌及相對(duì)應(yīng)的癌旁正常組織(注:正常組織距癌組織1cm,鏡下未見(jiàn)癌細(xì)胞、腺瘤性增生細(xì)胞等異常細(xì)胞)。采用免疫組化Max Vision TM染色法檢測(cè)DCLK1和Bmi-1在大腸癌中的表達(dá)狀況,統(tǒng)計(jì)分析兩者之間相關(guān)性及兩者與大腸癌臨床病理參數(shù)之間的關(guān)系。全部數(shù)據(jù)利用軟件SPSS20.0進(jìn)行統(tǒng)計(jì)學(xué)分析,計(jì)數(shù)資料采納2x檢驗(yàn),DCLK1和Bmi-1的相關(guān)性采用Spearman等級(jí)相關(guān)分析,定義差異有統(tǒng)計(jì)學(xué)意義是P0.05,差異性顯著則是P0.01!窘Y(jié)果】:1.DCLK1蛋白在大腸癌組織的陽(yáng)性表達(dá)率(68.9%)明顯超過(guò)癌旁正常組織(27.0%),黃色或棕黃色的染色免疫產(chǎn)物主要定位于細(xì)胞漿,呈顆粒狀;經(jīng)2x檢驗(yàn),2x=26.016,P0.01,差異性顯著;與大腸癌浸潤(rùn)深度、淋巴結(jié)轉(zhuǎn)移、Dukes分期存在顯著相關(guān)性(P0.05),差異有統(tǒng)計(jì)學(xué)意義;與患者的性別、年齡、腫瘤部位、腫瘤分化程度、腫瘤分型均無(wú)顯著相關(guān)性(P0.05)。2.Bmi-1蛋白在大腸癌組織的陽(yáng)性表達(dá)率(62.2%)明顯超過(guò)癌旁正常組織(23.0%),黃色、棕黃色或棕褐色的染色免疫產(chǎn)物主要定位在細(xì)胞核,少部分在細(xì)胞漿,呈顆粒狀,經(jīng)2x檢驗(yàn),2x=23.243,P0.01,差異性顯著;與腫瘤分化程度、浸潤(rùn)深度、淋巴結(jié)轉(zhuǎn)移、Dukes分期存在顯著相關(guān)性(P0.05),差異有統(tǒng)計(jì)學(xué)意義;與患者的性別、年齡、腫瘤部位、腫瘤分型均無(wú)顯著相關(guān)性(P0.05)。3.DCLK1與Bmi-1在大腸癌組織中表達(dá)水平之間的關(guān)系經(jīng)Spearman等級(jí)相關(guān)分析后結(jié)果顯示存在一定相關(guān)性,呈正相關(guān)(rs=0.259,P=0.026,P0.05),差異有統(tǒng)計(jì)學(xué)意義!窘Y(jié)論】:1.在大腸癌組織中DCLK1呈高表達(dá),與大腸癌浸潤(rùn)、淋巴結(jié)轉(zhuǎn)移、Dukes分期均存在顯著相關(guān)性。2.在大腸癌組織中Bmi-1呈高表達(dá),與大腸癌分化、浸潤(rùn)、淋巴結(jié)轉(zhuǎn)移、Dukes分期均存在顯著相關(guān)性。3.在大腸癌組織中DCLK1和Bmi-1的表達(dá)存在一定相關(guān)性,呈正相關(guān),提示DCLK1可能通過(guò)某種機(jī)制間接影響B(tài)mi-1的表達(dá),兩者可能協(xié)同參加大腸癌的發(fā)病、進(jìn)展、浸潤(rùn)及轉(zhuǎn)移;抑制兩者的表達(dá)可能會(huì)阻斷腫瘤的發(fā)生及發(fā)展;聯(lián)合檢測(cè)DCLK1和Bmi-1可能為大腸癌的臨床診治及預(yù)后評(píng)判提供新途徑。
[Abstract]:[objective]: to investigate the expression of DCLK1 and Bmi-1 in 74 cases of colorectal carcinoma. To understand the relationship between DCLK1, Bmi-1 and colorectal cancer. [methods] 74 patients with colorectal cancer from September 2015 to October 2016 were collected from affiliated Hospital of Yan'an University. The selected specimens included colorectal cancer and the corresponding adjacent normal tissue (note: normal tissue from cancer tissue 1 cm, no cancer cells under the microscope, adenomatous hyperplasia cells and other abnormal cells. Immunohistochemical Max Vision TM staining was used to detect the expression of DCLK1 and Bmi-1 in colorectal carcinoma, and the correlation between them and clinicopathological parameters was analyzed statistically. All the data were statistically analyzed by software SPSS20.0, and the correlation between DCLK1 and Bmi-1 was analyzed by using 2x test. Spearman grade correlation analysis was used to analyze the correlation between DCLK1 and Bmi-1. The difference in definition was P0.05, and the difference was P0.01.Results the positive expression rate of DCLK1 protein in colorectal carcinoma was significantly higher than that in adjacent normal tissues (P 0.05 and P 0.01.Results) the positive expression rate of DCLK1 protein in colorectal cancer tissues was significantly higher than that in adjacent normal tissues. The immunoreactive products of yellow or brown staining were mainly located in cytoplasm. The difference was significant with the depth of invasion of colorectal cancer and the Dukes stage of lymph node metastasis, the difference was statistically significant, and the difference was statistically significant with the sex, age, tumor location and tumor differentiation of the patients. The positive expression rate of Bmi-1 protein in colorectal carcinoma tissues was significantly higher than that in adjacent normal tissues. The immunoreactive products of yellow, brown or brown staining were mainly located in the nucleus, a few in the cytoplasm, and were granular. By 2x test, the difference was significant, and there was significant correlation with tumor differentiation, depth of invasion and Dukes stage of lymph node metastasis (P 0.05), sex, age and location of tumor. There was no significant correlation between tumor classification and the expression level of Bmi-1 in colorectal cancer tissues. The results of Spearman grade correlation analysis showed that there was a positive correlation between P0.05T and Bmi-1 in colorectal carcinoma. [conclusion] there was a significant difference between two groups. [conclusion] there is a significant difference in the expression of Bmi-1 in colorectal carcinoma. [conclusion] the expression level of DCLK1 in colorectal carcinoma is significantly higher than that in control group. [conclusion] there is a significant difference in the expression of Bmi-1 in colorectal carcinoma. High expression of DCLK1 was found in colorectal carcinoma, which was significantly correlated with invasion of colorectal carcinoma, lymph node metastasis and Dukes stage. High expression of Bmi-1 was found in colorectal carcinoma, which was significantly correlated with differentiation, invasion, lymph node metastasis and Dukes stage of colorectal carcinoma. There was a positive correlation between DCLK1 and Bmi-1 expression in colorectal carcinoma, which suggested that DCLK1 might indirectly affect the expression of Bmi-1 through some mechanism, and they might co-participate in the pathogenesis, progression, invasion and metastasis of colorectal carcinoma. The combination of DCLK1 and Bmi-1 may provide a new approach for clinical diagnosis and treatment and prognosis evaluation of colorectal cancer.
