【摘要】：背景與目的:卵巢黏液性腫瘤是卵巢上皮性腫瘤的主要亞型之一,其惡變機制目前尚未明確。泛素化是人體主要的翻譯后修飾機制,并與腫瘤密切相關。去泛素化酶可通過逆轉泛素化過程從而影響腫瘤的發(fā)生、發(fā)展。該研究旨在分析編碼人體最主要的去泛素化酶的癌基因泛素醛結合物1(OTU deubiquitinase,ubiquitin aldehyde binding 1,OTUB1)在卵巢黏液性腫瘤中的蛋白表達及臨床應用。方法:收集2010—2015年復旦大學附屬婦產科醫(yī)院病理科診斷的卵巢良性黏液性囊腺瘤、卵巢交界性黏液性腫瘤及卵巢原發(fā)性黏液性癌病例共90例,收集臨床病理資料,完成OTUB1的免疫組織化學法表達實驗并作統(tǒng)計分析。結果:90例卵巢黏液性腫瘤中,卵巢良性黏液性囊腺瘤有14例,卵巢交界性黏液性腫瘤有17例,卵巢原發(fā)性黏液性癌有59例。OTUB1在卵巢原發(fā)性黏液性癌的蛋白表達率及陽性程度顯著高于卵巢良性黏液性囊腺瘤(P0.01);OTUB1在卵巢交界性黏液性腫瘤中上皮內癌的陽性率顯著高于腸型交界性黏液性腫瘤(P0.01);隨著FIGO分期增高,OTUB1的陽性率及陽性程度增加(P0.05);OTUB1在輸卵管有累及的患者中表達率及陽性程度均高于未累及者(P0.05);OTUB1在子宮和大網(wǎng)膜累及的患者中陽性率及陽性程度均高于未累及者(P0.05);OTUB1在有淋巴結轉移的患者中表達率及陽性程度均高于無轉移者(P0.05)。結論:OTUB1在卵巢原發(fā)性黏液性癌中的蛋白表達水平明顯高于卵巢良性黏液性囊腺瘤,并且和FIGO分期及腫瘤侵襲轉移性高度相關。在臨床病理中可將OTUB1作為卵巢黏液性腫瘤惡變與否的評價指標及腫瘤進展的輔助判別指標。
[Abstract]:Background & AIM: ovarian mucinous tumor is one of the main subtypes of epithelial ovarian tumor. Ubiquitin is the main posttranslational modification mechanism in human body and is closely related to tumor. De-ubiquitin enzyme can influence the development of tumor by reversing the process of ubiquitin. The aim of this study was to analyze the protein expression and clinical application of 1(OTU ubiquitin aldehyde binding 1 OTUB1, a major oncogene ubiquitin aldehyde conjugate encoding the most important deubiquiase in human ovarian mucinous tumors. Methods: a total of 90 cases of benign ovarian mucinous cystadenoma, borderline ovarian mucinous tumor and primary ovarian mucinous carcinoma were collected from the Department of Pathology, Department of Pathology, affiliated Obstetrics and Gynecology Hospital of Fudan University from 2010 to 2015. The clinicopathological data were collected. The immunohistochemical expression of OTUB1 was carried out and statistical analysis was made. Results among 90 cases of ovarian mucinous tumors, 14 cases were benign ovarian mucinous cystadenoma and 17 cases were borderline ovarian mucinous tumors. The protein expression and positive degree of OTUB1 in ovarian primary mucinous carcinoma were significantly higher than that of benign ovarian mucinous cystadenoma P0.01OTUB1 in ovarian borderline mucinous tumor. The positive rate of OTUB1 in ovarian borderline mucinous carcinoma was significantly higher than that in ovarian benign mucinous cystadenoma. The positive rate and degree of OTUB1 in patients with fallopian tube involvement were higher than those in patients with tubal involvement, and the positive rate and degree of OTUB1 in uterus and omentum were higher than those in patients without tubal involvement, and the positive rate and degree of OTUB1 in uterus and omentum were higher than those in patients with intestinal borderline mucinous neoplasms (P 0.01), and the positive rate and degree of OTUB1 in uterus and greater omentum were higher than those in patients with fallopian tube involvement. The positive rate and degree of OTUB1 in patients with lymph node metastasis were higher than those without metastasis. Conclusion the protein expression level of OTUB1 in primary mucinous ovarian carcinoma is significantly higher than that in benign ovarian mucinous cystadenoma, and is highly correlated with FIGO stage and tumor invasion and metastasis. In clinicopathology, OTUB1 can be used as an index to evaluate the malignancy of ovarian mucinous tumor and as an auxiliary discriminant index of tumor progression.
