【摘要】：目的探討西黃膠囊(人工牛黃、人工麝香、乳香、沒藥)對急性放射性口腔黏膜炎的防治作用及相關(guān)機制。方法將80例接受根治性放療的鼻咽癌患者隨機分為2組,給藥組(照射+西黃膠囊)和對照組(單純照射)。依據(jù)急性放射性黏膜損傷分級標準比較2組開始出現(xiàn)黏膜炎的時間和耐受劑量。并用ELISA法檢測唾液炎性因子TNF-α和IL-6水平。結(jié)果與單純照射組比較,給藥組出現(xiàn)黏膜炎時間明顯偏晚,放療耐受劑量顯著偏高(t=2.180、12.930,P0.05);DT=40、70 Gy時,給藥組Ⅲ、Ⅳ度口腔黏膜炎發(fā)生率均低于單純照射組(Z=3.661、4.270,P0.01);放療前(DT=0 Gy),2組炎性因子TNF-α和IL-6質(zhì)量濃度無差別(t=1.010、1.469,P0.05);炎性因子水平隨著放療劑量的增加而升高,DT=40 Gy時達峰值,但給藥組TNF-α和IL-6質(zhì)量濃度均顯著低于單純照射組(t=8.305、6.069,P0.05);DT=70 Gy時,炎性因子均降至低于治療前水平且給藥組明顯低于單純照射組(t=3.835、2.488,P0.05)。結(jié)論西黃膠囊能夠延緩并減輕急性放射性口腔黏膜炎,提高放療耐受劑量,可能與抑制炎性介質(zhì)TNF-α和IL-6的釋放有關(guān)。
[Abstract]:Objective to investigate the preventive and therapeutic effects of Xihuang capsule (artificial bezoar, artificial musk, frankincense, myrrh) on acute radioactive oral mucositis.Methods 80 patients with nasopharyngeal carcinoma receiving radical radiotherapy were randomly divided into two groups: the control group and the control group.According to the classification criteria of acute radiation mucosal injury, the time and tolerance dose of mucositis were compared between the two groups.The levels of TNF- 偽 and IL-6 were detected by ELISA.Results compared with the irradiation group, the time of mucositis in the administration group was obviously late, the dose of radiation tolerance was significantly higher than that in the control group, and the dosage of radiation tolerance was significantly higher than that in the control group.The incidence of oral mucositis in grade 鈪，
