血清Cyfra21-1、SCCA和TK1基線(xiàn)水平與肺癌預(yù)后關(guān)系的巢式病例對(duì)照研究

【摘要】:目的:探討Cyfra21-1、SCCA和TK1水平與肺癌預(yù)后關(guān)系。方法:采用巢式病例對(duì)照研究方法,以2010年1月至2013年1月首診無(wú)遠(yuǎn)處轉(zhuǎn)移的721肺癌患者為基線(xiàn)隊(duì)列,以隨訪(fǎng)期間(2年)死亡或腦(或多處)轉(zhuǎn)移的364例患者為病例組,其他357例納入對(duì)照組。檢測(cè)兩組血清Cyfra21-1、SCCA和TK1水平,分析三者與肺癌預(yù)后關(guān)系。結(jié)果:兩組間體重指數(shù)、吸煙情況、病理類(lèi)型、臨床分期、淋巴轉(zhuǎn)移、合并慢性疾病等,差異有統(tǒng)計(jì)學(xué)意義(P0.05),兩組胸腔積液、綜合治療情況等,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);兩組Cyfra21-1、SCCA和TK1基線(xiàn)水平,差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組間Cyfra21-1、SCCA、SCCA和TK1的分布情況,差異有統(tǒng)計(jì)學(xué)意義(P0.05);不同臨床分期肺癌患者Cyfra21-1基線(xiàn)水平比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),SCCA和TK1差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);不同病理類(lèi)型肺癌患者Cyfra21-1、SCCA基線(xiàn)水平比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),TK1差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。多因素Logistic分析顯示Cyfra21-1、SCCA和TK1水平肺癌預(yù)后不良的OR值(95%CI)分別為7.235(1.674~14.613)、5.009(0.973~10.778)、5.816(0.879~16.235)。結(jié)論:Cyfra21-1基線(xiàn)水平與肺癌預(yù)后有關(guān),可增加預(yù)后不良的危險(xiǎn)性,SCCA和TK1水平變化與肺癌預(yù)后無(wú)關(guān)。
[Abstract]:Objective: to investigate the relationship between the levels of SCCA and TK1 in Cyfra21-1 and the prognosis of lung cancer.Methods: a nested case-control study was conducted with a baseline cohort of 721 lung cancer patients with no distant metastasis from January 2010 to January 2013, and 364 patients who died or had brain (or multiple) metastases during the follow-up period (2 years).Other 357 cases were included in the control group.The serum levels of Cyfra21-1 and TK1 were measured, and the relationship between them and the prognosis of lung cancer was analyzed.Results: there were significant differences in body mass index (BMI), smoking status, pathological type, clinical stage, lymphatic metastasis and chronic disease between the two groups (P 0.05).There was no significant difference between the two groups (P 0.05); the baseline level of Cyfra21-1 and TK1 was statistically significant (P 0.05); the distribution of SCCA and TK1 in the two groups was statistically significant (P 0.05); the baseline level of Cyfra21-1 in patients with different clinical stages of lung cancer was higher than that in the control group.There was no significant difference between SCCA and TK1 in patients with different pathological types of lung cancer (P 0.05), but there was no significant difference in baseline level of Cyfra21-1 and TK1 in patients with different pathological types of lung cancer (P 0.05).Multivariate Logistic analysis showed that the OR value of poor prognosis of lung cancer in Cyfra21-1SCCA and TK1 was 7.235 ~ 1.674 ~ 14.613 ~ 5.009 ~ 0.973 ~ (10.778) and 5.816 ~ (0.879) ~ (16.235) respectively.Conclusion the baseline level of Cyfra21-1 is related to the prognosis of lung cancer, and the risk of poor prognosis is increased. The changes of SCCA and TK1 levels are not related to the prognosis of lung cancer.
